老年髋部骨折围手术期中医综合护理临床观察

Comprehensive Nursing of Traditional Chinese Medicine in Perioperative Period of Senile Patients with Hip Fracture

目的探究中医综合护理对老年髋部骨折围手术期的临床效果。方法回顾性分析2019年1月—2021年12月江西中医药大学附属医院收治的老年髋部骨折患者90例,分为对照组45例(常规护理),观察组45例(在常规护理基础上行中医综合护理)。收集患者术前、术后7 d视觉模拟量表(VAS)评分及Harris髋关节(HHS)评分、住院时间、下床锻炼时间、护理满意度、并发症发生率、生活质量以评价临床疗效。结果观察组并发症发生率、下床锻炼时间、住院时间、VAS评分均低于对照组(P<0.05);观察组HHS评分、生活质量评分、护理总满意度高于对照组(P<0.05);观察组临床疗效优良率为88.89%(40/45),高于对照组的66.67%(30/45)。结论对老年髋部骨折围手术期患者实行中医综合护理,可有效提升患者护理满意度、生活质量及髋关节功能,同时减少并发症发生率、缩短住院天数。

Objective To explore the clinical effect of comprehensive nursing of traditional Chinese medicine on perioperative period of senile hip fracture.Methods Retrospective analysis was made on 90 elderly patients with hip fracture admitted to Affiliated Hospital of Jiangxi University of Chinese Medicine from January 2019 to December 2021.They were divided into the control group of 45 patients received routine care and observation group of 45 cases received comprehensive nursing care of traditional Chinese medicine on the basis of routine nursing care.Visual analogue scale(VAS)and Harris Hip Score(HHS),length of hospital stay,time of getting out of bed exercise,nursing satisfaction,incidence of complications and quality of life were collected before and 7 days after operation to evaluate the clinical efficacy.Results The incidence of complications,exercise time,hospital stay and VAS score in observation group were lower than those in control group(P<0.05).HHS score,quality of life score and nursing total satisfaction in observation group were higher than those in control group(P<0.05).The excellent and good rate of clinical efficacy in the observation group was 88.89%(40/45),which was higher than that in the control group(66.67%,30/45).Conclusion Perioperative comprehensive nursing of traditional Chinese medicine for senile patients with hip fracture can effectively improve nursing satisfaction,quality of life and hip function,reduce the incidence of complications and shorten the length of hospital stay.
