输尿管软镜碎石术联合负压吸引输尿管通道鞘治疗上尿路结石的效果

Effect of RIRS combined with NPUAS in treatment of upper urinary tract calculi

目的探讨输尿管软镜碎石术(RIRS)联合负压吸引输尿管通道鞘(NPUAS)治疗上尿路结石的效果。方法选取我院2020年2月至2022年5月收治的上尿路结石患者109例,随机分为对照组54例和观察组55例。对照组采用常规RIRS治疗,观察组采用RIRS联合NPUAS治疗,观察比较两组患者的手术相关指标、结石清除率及并发症发生情况。结果观察组的手术时间、住院时间短于对照组,术后血红蛋白水平高于对照组,白细胞(WBC)、降钙素原(PCT)水平均低于对照组,差异均有统计学意义(均P<0.05)。观察组的结石清除率高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,观察组(5.46%)明显低于对照组(18.51%),差异有统计学意义(P<0.05)。结论RIRS联合NPUAS治疗上尿路结石的效果显著,结石排除率高,并发症少,值得临床应用。

Objective To investigate the effect of ureteroscopic lithotripsy(RIRS)combined with negative pressure suction ureteral catheter sheath(NPUAS)in the treatment of upper urinary tract calculi.Methods According to the computer grouping method,109 patients with upper urinary tract calculi admitted in our hospital from February 2020 to May 2022 were prospectively selected and divided into two groups.The control group(54 cases)was treated with conventional RIRS,and the observation group(55 cases)was treated with RIRS combined with NPUAS.The operation related indexes,stone clearance rate,renal function and complication rate of the two groups were observed.Results The operation time and hospitalization time in the observation group were shorter than those in the control group,and the hemoglobin decrease,WBC and PCT levels in the observation group were lower than those in the control group(all P<0.05);The stone clearance rate in the observation group was higher than that in the control group(P<0.05);The incidence of complications in the observation group(5.46%)was lower than that in the control group(18.51%)(P<0.05).Conclusion The application of RIRS combined with NPUAS in the removal of upper urinary tract stones has a significant effect,which can shorten the operation time,reduce the number of hemoglobin drops,WBC and PCT levels,reduce complications,shorten the hospital stay,and do not affect the recovery of renal function.
