摘要
目的分析MRI与CT检查对肝癌的诊断价值。方法回顾性分析2019年6月至2021年2月本院收治的52例肝癌患者的临床资料作为肝癌组,另选取同期本院收治的52例肝硬化患者的临床资料作为肝硬化组,所有患者均行MRI及CT检查。比较两组CT定量参数[血流灌注量(HAF)、灌注指数(HPI)、门静脉血流灌注量(HPF)]、MRI峰值时间与信号增强比率,比较MRI及CT对肝癌诊断价值。结果肝癌组HAF、HPI、HPF水平均低于肝硬化组,差异有统计学意义(P<0.05)。肝癌组MRI峰值时间短于肝硬化组,信号增强比率均低于肝硬化组,差异有统计学意义(P<0.05)。MRI对肝癌诊断的灵敏度、特异度、准确度为98.08%、100.00%、99.04%;CT对肝癌诊断的灵敏度、特异度、准确度为94.23%、98.08%、96.15%;MRI与CT的灵敏度、特异度、准确度比较差异无统计学意义。结论MRI、CT对肝病理特征的判断均有自身价值及特点,临床可根据肝癌患者实际情况采取检查手段。
Objective To analyze the diagnostic value of MRI and CT in liver cancer.Methods The clinical data of 52 patients with liver cancer admitted to our hospital from June 2019 to February 2021 were retrospectively analyzed as the liver cancer group,and the clinical data of 52 patients with cirrhosis admitted to our hospital during the same period were selected as the cirrhosis group,all patients underwent MRI and CT examinations.CT quantitative parameters(hepatic artery flow[HAF],hepatic perfusion index[HPI],hepatic plasma flow(HPF)),MRI peak time and signal enhancement ratio were compared between the two groups,and the diagnostic value of MRI and CT in liver cancer were compared.Results The levels of HAF,HPI and HPF in the liver cancer group were lower than the cirrhosis group,and the differences were statistically significant(P<0.05).The MRI peak time in liver cancer group was shorter than the cirrhosis group,and the ratio of signal enhancement was lower than the cirrhosis group,the differences were statistically significant(P<0.05).The sensitivity,specificity and accuracy of MRI in the diagnosis of liver cancer were 98.08%,100.00%and 99.04%;the sensitivity,specificity and accuracy of CT in the diagnosis of liver cancer were 94.23%,98.08%and 96.15%;there were no significant differences in sensitivity,specificity and accuracy between MRI and CT.Conclusion MRI and CT have their own value and characteristics in judging the characteristics of liver disease,and can be examined according to the actual situation of patients with liver cancer.
