外固定支架与有限内固定联合治疗桡骨远端粉碎性骨折的效果研究

Effect of External Fixation with Limited Internal Fixation in the Treatment of Comminuted Fracture of Distal Radius

目的探讨外固定支架与有限内固定联合治疗桡骨远端粉碎性骨折的效果。方法选取2018年7月—2021年6月新泰市人民医院收治的62例桡骨远端粉碎性骨折患者,按照随机数表法分为两组,每组31例。对照组患者应用钢板内固定术治疗,研究组应用外固定支架与有限内固定联合治疗。比较两组治疗的优良率、术后3个月时腕关节活动度,以及术后并发症情况。结果研究组治疗的优良率(93.55%)显著高于对照组(70.97%),差异有统计学意义(χ^(2)=5.415,P<0.05)。研究组术后3个月时尺偏角(22.65±0.48)°、掌曲角(45.52±1.02)°、背伸角(68.12±1.00)°、旋后角(74.65±1.48)°、旋前角(69.50±1.85)°均大于对照组(16.42±0.58)°、(39.54±0.98)°、(40.20±1.15)°、(45.65±1.38)°、(62.48±1.67)°,差异有统计学意义(t=46.074、23.539、102.004、79.793、15.683,P<0.05)。研究组术后并发症发生率(3.23%)显著低于对照组(25.81%),差异有统计学意义(χ^(2)=4.679,P<0.05)。结论外固定支架联合有限内固定治疗能够改善桡骨远端粉碎性骨折患者的腕关节活动度,效果显著,安全性佳。

Objective To investigate the effect of external fixation and limited internal fixation in the treatment of comminuted fracture of distal radius.Methods 62 patients with comminuted distal radius fracture admitted to Xintai People's Hospital from July 2018 to June 2021 were selected,and divided into two groups according to a random number table,thirty-one patients in each group.The control group was treated with internal fixation with steel plate,while the study group was treated with external fixation stent and limited internal fixation.The rate of good and good treatment,wrist motion at 3 months after surgery,and postoperative complications were compared between the two groups.Results The excellent and good rate of the study group(93.55%)was significantly higher than that of the control group(70.97%),and the difference was statistically significant(χ^(2)=5.415,P<0.05).The deviation angle(22.65±0.48)°,palmar curvature angle(45.52±1.02)°,dorsoextension angle(68.12±1.00)°,supination angle(74.65±1.48)°,pronation angle(69.50±1.85)°in the study group were higher than the control group(16.42±0.58)°,(39.54±0.98)°,(40.20±1.15)°,(45.65±1.38)°,(62.48±1.67)°,and the difference was statistically significant(t=46.074,23.539,102.004,79.793,15.683,P<0.05).The incidence of postoperative complications was significantly lower in the study group(3.23%)than in the control group(25.81%),and the difference was statistically significant(χ^(2)=4.679,P<0.05).Conclusion The combination of external fixation with limited internal fixation can improve the range of motion of the wrist in patients with comminuted fracture of the distal radius,with significant effect and good safety.
