心脏超声在慢性心力衰竭诊断中的临床价值

Clinical Value of Cardiac Ultrasound in the Diagnosis of Chronic Heart Failure

目的分析心脏超声在慢性心力衰竭(简称慢性心衰)诊断中的价值。方法选取2020年10月—2022年6月枣庄市立医院疑似80例慢性心力衰竭,进行心脏超声检查,以经冠状动脉造影作为诊断金标准,分析检查结果间差异。结果经冠状动脉造影阳性率为48.75%,心脏超声检查阳性率为50.00%。心脏超声检查的敏感度为100%、特异度为97.56%、准确性为98.75%、误诊率为2.43%、漏诊率为0、阳性预测值为97.50%、阴性预测值为100%。采用ROC曲线分析心脏超声对慢性心力衰竭诊断价值,结果显示,ROC曲线下面积为0.975(95%CI:0.935~1.000,P<0.05)。结论心脏超声对慢性心力衰竭具有较高的诊断价值,值得临床推广。

Objective To analyze the value of cardiac ultrasound in the diagnosis of chronic heart failure(CCHF).Methods Eighty suspected cases of chronic heart failure in Zaozhuang Municipal Hospital from October 2020 to June 2022 were selected for cardiac ultrasound examination.Coronary angiography was used as the gold standard for diagnosis,and the differences among the results were analyzed.Results Coronary angiography showed that the positive rate was 48.75%,and the positive rate of cardiac ultrasound was 50.00%.The sensitivity,specificity,accuracy,misdiagnosis rate,positive predictive value and negative predictive value were 100%,97.56%,98.75%,2.43%,0,97.50%and 100%respectively.ROC curve was used to analyze the diagnostic value of cardiac ultrasound in chronic heart failure.The results showed that the area under ROC curve was 0.975(95%CI:0.935-1.000,P<0.05).Conclusion Cardiac ultrasound has a high diagnostic value for chronic heart failure and is worthy of clinical promotion.
