血清白介素-10与慢阻肺患者肺功能之间的关系

Relationship between serum interleukin-10 and lung function in patients with chronic obstructive pulmonary disease

目的 探讨血清白介素-10(IL-10)与稳定期慢性阻塞性肺疾病(慢阻肺)患者肺功能之间的关系。方法 选取2020年11月—2021年5月于安徽医科大学第三附属医院呼吸与危重症医学科门诊就诊的慢阻肺稳定期患者124例,所有患者完善基本临床信息调查,于第二日早晨空腹检测血清IL-10,并完善肺功能检查。比较血清IL-10与肺通气功能、小气道阻塞程度及弥散功能之间的关系。结果 血清IL-10含量在轻中度慢阻肺患者中明显大于重度与极重度患者(P<0.001)。IL-10与吸烟指数呈负相关(r=-0.188,P=0.037),与FEV_(1)/FVC(r=0.490,P<0.001)、FEV_(1)%pred(r=0.601,P<0.001)、FVC%pred(r=0.392,P<0.001)呈正相关,与小气道指标FEF_(50%)pred(r=0.337,P<0.001)、FEF_(75%)pred(r=0.439,P<0.001)、MMEF%pred(r=0.384,P<0.001)呈正相关。不同弥散功能组别之间,IL-10无明显差别(P>0.05)。结论 IL-10作为一种重要的抗炎因子,与气流阻塞程度及小气道功能有关。其可能在慢阻肺患者气道和肺功能的保护方面起着重要的作用。

Objective To investigate the relationship between serum interleukin-10(IL-10)and lung function in patients with stable chronic obstructive pulmonary disease(COPD).Methods From November 2020 to May 2021,124 patients with stable COPD who were admitted to the Department of Respiratory and Critical Care Medicine of the Third Affiliated Hospital of Anhui Medical University were selected.All patients completed a basic clinical information survey,fasting serum IL-10 was detected on the morning of the second day,and a lung function examination was completed.To compare the relationship between serum IL-10 and pulmonary ventilation function,degree of small airway obstruction and diffusion function.Results The serum IL-10 level in patients with mild and moderate COPD was significantly higher than that in patients with severe and extremely severe COPD(P<0.001).IL-10 was negatively correlated with smoking index(r=-0.188,P=0.037),and was positively correlated with FEV 1/FVC(r=0.490,P<0.001),FEV 1%pred(r=0.601,P<0.001),FVC%pred(r=0.392,P<0.001),FEF 50%pred(r=0.337,P<0.001),FEF 75%pred(r=0.439,P<0.001),MMEF%pred(r=0.384,P<0.001).There was no significant difference in IL-10 between the diffusion-impaired groups(P>0.05).Conclusion IL-10,as an important anti-inflammatory factor,is related to the degree of airflow obstruction and small airway function.It may play an important role in the protection of lung function in COPD patients.
