韦氏儿童智力量表第四版中文版在儿童智力残疾评定中的应用研究

Study on the Application of Wechsler Intelligence Scale for Children Fourth Edition Chinese Version in the Assessment of Children's Intellectual Disability

目的:探讨韦氏儿童智力量表第四版(WISC-Ⅳ)中文版简式版本在智力残疾评定中的应用效果。方法:选取辽宁省残疾人服务中心2019年1月—2022年12月收治的智力残疾儿童53例,并通过WISC-Ⅳ完整版测验进行检测。从完整版数据里分别选取四分测验、五分测验和六分测验的数据,采用离差智商法推算出总智商,比较完整版智商及三种简式智商(四合一、五合一、六合一)评分的准确性。结果:三种简式智商和完整版智商评分,差异均无统计意义(P>0.05)。简式四分测验和完整版WISC-Ⅳ在智商评残中的评分Kappa值为0.783,存在良好一致性;简式五分测验和完整版WISC-Ⅳ在智商评残中的评分Kappa值为0.822,具有较高一致性;简式六分测验与完整版WISC-Ⅳ在智商评残中的评分Kappa值是0.671,一致性尚可。结论:简式四分测试用时更少,临床使用更加方便,值得推广,可作为儿童智力评残的评估工具,或作为患儿配合度较差、时间紧急等情况下智力测评的主要参考指标。

Objective:To explore the application effect of the simplified version of Wechsler intelligence scale for children fourth edition(WISC-Ⅳ)Chinese version in the assessment of intellectual disability.Method:A total of 53 children with intellectual disabilities who were admitted to the Liaoning Disabled Persons Service Center from January 2019 to December 2022 were selected,and tested by the WISC-Ⅳ full version test.The data of the fourpoint test,five-point test and six-point test were selected from the full version data,and the total IQ was calculated adopted to the method of deviation IQ,and the accuracy of the full version IQ and the three simplified IQ(four in one,five in one,six in one)scores were compared.Result:There were no significant differences between the three simplified IQ and the full version IQ scores(P>0.05).The Kappa value of the consistency between the simplified four-point tests and the full version of WISC-Ⅳ in the assessment of IQ disability was 0.783,which still had good consistency;the consistency Kappa value of the simplified five-point tests and the complete version of WISC-Ⅳ in IQ assessment was 0.822,showed a high consistency.The consistency Kappa value between the simplified sixpoint test and the full version of WISC-Ⅳ in the assessment of IQ disability was 0.671,with acceptable accuracy.Conclusion:The simplified four-point test is worth promoting and can be used as an evaluation tool for children's intelligence disability assessment with less time and more convenient for clinical use,or as the main reference index for intelligence assessment in children with poor cooperation and time emergency.
