摘要
目的:探讨多频振动治疗仪联合分期运动在小儿支气管肺炎护理中康复效果的观察,旨在为小儿支气管肺炎康复护理方案制订提供参考。方法:采用前瞻性随机对照研究,选择2020年4月—2022年4月抚州市临川区第一人民医院收治的86例支气管肺炎患儿作为研究对象,按照随机数字表法分为三组,A组28例,B组29例,C组29例。A组实施常规护理干预,B组在常规护理基础上使用多频振动治疗仪进行干预,C组在B组基础上联合分期运动干预。比较三组症状消失时间、住院时间、肺部并发症情况、干预前后潮气肺功能指标[潮气量(VT)、吸呼比(TI/TE)、达峰容积比(VPEF/VE)、达峰时间比(TPTEF/TE)]及干预1、3、5、7 d的排痰量。结果:C组症状消失时间及住院时间均最短,其次为B组,A组均最长,差异均统计学意义(P<0.05)。C组干预1、3 d排痰量最多,干预5、7 d排痰量最少,三组组间、时点、组间时点交互比较,差异均有统计学意义(P<0.05)。干预后,C组VT、TI/TE、VPEF/VE、TPTEF/TE最高,其次为B组,A组最低,差异均有统计学意义(P<0.05)。C组肺部并发症总发生率最低,其次为B组,A组最高,差异有统计学意义(P<0.05)。结论:多频振动治疗仪联合分期运动能有效改善支气管肺炎患儿临床症状,促进痰液排出,提高肺功能,并减少肺部并发症发生。
Objective:To explore the rehabilitation effect of multi-frequency vibration therapeutic apparatus combined with staged exercise in nursing of children with bronchopneumonia,so as to provide reference for the formulation of rehabilitation nursing plan for children with bronchopneumonia.Method:A prospective randomized controlled study was conducted,a total of 86 children with bronchopneumonia who were admitted to the First People's Hospital of Linchuan District,Fuzhou from April 2020 to April 2022 were selected as the research objects.According to the random number table method,they were divided into three groups,group A with 28 cases,group B with 29 cases,and group C with 29 cases.Group A was given routine nursing intervention,group B was given multi�frequency vibration therapeutic instrument intervention on the basis of routine nursing,and group C was given staged exercise intervention on the basis of group B.The symptom disappearance time,hospitalization time,pulmonary complications,tidal lung function indexes[tidal volume(VT),ratio of inspiratory to expiratory time(TI/TE),ratio of time to peak tidal expiratory flow to total expiratory time(VPEF/VE),ratio of time to peak tidal expiratory flow to total expiratory time(TPTEF/TE)]before and after intervention,and sputum excretion on the 1st,3rd,5th and 7th day of intervention were compared among the three groups.Result:The symptom disappearance time and hospitalization time of group C were the shortest,followed by group B,and group A were the longest,the differences were significant(P<0.05).The amount of sputum excretion in group C was the highest on the 1st and 3rd day of intervention,and the least on the 5th and 7th day of intervention.There were statistical significant differences in the amount of sputum excretion among the three groups,time points and interaction between groups at time points(P<0.05).After intervention,VT,TI/TE,VPEF/VE and TPTEF/TE were the highest in group C,followed by group B,and the lowest in group A,the differences were statistically significant(P<0.05).The total incidence of pulmonary complications in group C was the lowest,followed by group B,and group A was the highest,with statistical significant difference(P<0.05).Conclusion:Multi-frequency vibration therapeutic apparatus combined with staged exercise can effectively improve the clinical symptoms of children with bronchopneumonia,promote sputum discharge,improve lung function,and reduce the incidence of pulmonary complications.
