老年住院COPD病人合并肌少症情况及其影响因素研究

Condition and influencing factors in elderly hospitalized patients with COPD complicated with sarcopenia

目的:分析长沙市某三级甲等医院老年住院慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)病人合并肌少症情况及其影响因素。方法:采用便利抽样法,选取2020年1月—12月长沙市某三级甲等综合医院呼吸与危重症医学科的257例老年COPD病人为研究对象,根据是否合并肌少症分为肌少症组和非肌少症组,采用病人一般资料调查表、肌少症筛查量表、改良版微型营养评定量表(Modified Mini-Nutrition Assessment,MMNA)进行调查,并收集病人住院时的其他相关指标。采用Logistic回归分析,探讨COPD合并肌少症发生的影响因素。结果:COPD合并肌少症病人68例(26.46%),Logistic回归分析结果显示,年龄、吸烟史、饮酒史、住院次数、性别、COPD病程、跌倒史、营养不良、清蛋白是COPD病人合并肌少症病人的影响因素(P<0.05)。结论:年龄、吸烟史、饮酒史、住院次数、性别、COPD病程、跌倒史、营养不良、清蛋白会增加COPD病人合并肌少症的风险,应根据风险因素针对性干预。

Objective:To investigate the condition and its influencing factors in the elderly patients with chronic obstructive pulmonary disease(COPD)combined with sarcopenia in a tertiary A hospital in Changsha City.Methods:A total of 257 elderly COPD patients from the respiratory and critical care medicine Department of a Grade A general hospital in Changsha City from January to December 2020 were selected by convenience sampling method.They were divided into sarcopenia group and non-sarcopenia group according to whether they were combined with sarcopenia.Patient general information Questionnaire,Sarcopenia Screening Scale,Modified Mini-Nutrition Assessment(MMNA)were used to investigate the patients,and other relevant indicators were collected during hospitalization.Binary Logistic regression analysis was used to investigate the influencing factors of COPD combined with sarcopenia.Results:A total of 68 patients(26.46%)were COPD patients with sarcopenia.Binary Logistic regression analysis showed that age,smoking history,drinking history,hospitalization times,gender,COPD course,fall history,malnutrition and albumin were the influencing factors of COPD patients with sarcopenia(P<0.05).Conclusions:Age,smoking history,drinking history,hospitalization frequency,gender,COPD course,fall history,malnutrition and albumin can increase the risk of COPD patients with sarcopenia,interventions should be targeted according to risk factors.
