老年髋关节置换术后应用双侧鼻导管输氧配合呼吸功能训练法对患者谵妄及睡眠障碍的预防性护理效果

Preventive Effect of Oxygen Therapy via Bilateral Nasal Catheter Combined with Respiratory Function Training on Delirium and Sleep Disorders in Elderly Patients after Hip Replacement

目的:探究老年髋关节置换术后应用双侧鼻导管输氧配合呼吸功能训练法对患者谵妄及睡眠障碍的预防性护理效果.方法:选取2018年9月至2019年12月在本院接受治疗的老年髋关节置换患者72例,采用随机数表法分为观察组(n=35,应用双侧鼻导管输氧配合呼吸功能训练法护理干预)和对照组(n=37,仅采用双侧鼻导管输氧常规护理).采用谵妄评分量表(CAM)评定患者的谵妄状况,观察比较两组患者谵妄发生率、谵妄的持续时间以及住院时间;治疗前后采用匹兹堡睡眠质量指数(PSQI)量表评定两组患者的睡眠质量;并采用"患者对护理方式的满意情况调查表"对两组患者进行满意度调查.结果:观察组患者谵妄发生率、谵妄症状持续时间、住院时间低于对照组,差异有统计学意义(P<0.05);护理后,两组睡眠质量评分均下降,且观察组低于对照组,差异有统计学意义(P<0.05);观察组的总满意度高于对照组,差异有统计学意义(P<0.05).结论:针对老年髋关节置换术患者应用双侧鼻导管输氧配合呼吸功能训练法能够有效降低谵妄发生率和持续时间,提高睡眠质量,患者对该护理模式的满意度高,临床可根据实际情况择优选择.

Objective:To investigate the preventive effect of oxygen therapy via bilateral nasal catheter combined with respiratory function training on delirium and sleep disorders in elderly patients after hip replacement.Methods:A total of 72 elderly patients who underwent hip replacement in the hospital from September 2018 to December 2019 were randomly divided into the observation group(n=35)and the control group(n=37).The observation group received oxygen therapy via bilateral nasal catheter combined with respiratory function training after hip replacement,while the control group received oxygen therapy via bilateral nasal catheter alone.The incidence and duration of delirium,the length of hospital stay,sleep quality[Pitsburgh Sleep Quality Index(PSQI)]and satisfaction were compared between the groups.Results:The incidence and duration of delirium,and hospital stay in the observation group were lower/shorter than those in the control group(P<0.05).After nursing,sleep quality scores of both groups decreased,and the observation group had lower scores than the control group(P<0.05).The total satisfaction of observation group was higher than that of the control group(P<0.05).Conclusion:Oxygen therapy via bilateral nasal catheter combined with respiratory function training can effectively reduce the incidence of delirium,shorten the duration of delirium,and improve sleep quality and satisfaction of elderly patients after hip replacement.
