摘要
目的:探讨早期标准化肠内营养管理对胃癌根治术患者术后快速恢复的影响.方法:选取2019年1月至2020年7月期间本院接诊的拟行胃癌根治术治疗的患者64例,随机分为观察组和对照组,各32例.两组均给予早期肠内营养支持治疗,其中观察组采取早期标准化肠内营养管理,对照组采取常规管理.统计两组患者首次排气时间、术后住院时间及住院花费;比较两组治疗7d后营养状态、免疫功能及术后并发症发生率.结果:观察组首次排气时间、术后住院时间均短于对照组,住院花费少于对照组,差异均有统计学意义(P<0.05).术后7d,观察组血清白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)水平均高于对照组,差异有统计学意义(P<0.05).术后7d,观察组CD3^(+)T、CD4^(+)T、CD16^(+)CD56^(+)NK细胞比例均高于对照组,差异有统计学意义(P<0.05).比较两组术后并发症发生率显示,观察组低于对照组,差异有统计学意义(P<0.05).结论:对胃癌根治术患者实施早期标准化肠内营养管理,可促进患者术后快速恢复,提高营养状态及免疫能力,降低术后并发症的发生率.
Objective:To explore the effect of early standardized enteral nutrition management on rapid recovery of patients undergoing radical gastrectomy for gastric cancer.Methods:Sixty-four patients undergoing radical gastrectomy in our hospital from January 2019 to July 2020 were selected and randomly divided into an observation group and a control group,each with 32 cases.Both groups were given early enteral nutrition support treatment,in which the observation group was given early standardized enteral nutrition management and the control group was given routine management only.The first exhaust time,postoperative hospitalization time and hospitalization cost of the two groups were counted.The nutritional status,immune function and postoperative complications were compared between the two groups after 7 days of treatment.Results:The first exhaust time,postoperative hospitalization time and hospitalization cost of the observation group were shorter than those of the control group(P<0.05).Seven days after operation,the levels of serum albumin(ALB),prealbumin(PA)and transferrin(TRF)in the observation group were higher than those in the control group(P<0.05).Seven days after operation,the ratio of CD3+T,CD4+T,CD16+CD56+NK cells in the observation group was higher than that in the control group(P<0.05).Comparing the incidence of postoperative complications between the two groups,the observation group was lower than the control group(P<0.05).Conclusion:Early standardized enteral nutrition management for patients undergoing radical gastrectomy can promote their rapid recovery,improve their nutritional status and immunity,and reduce the incidence of postoperative complications.
