循证护理在新生儿脐静脉置管术围术期中的应用效果

Application Effect of Evidence-Based Nursing in Neonates During Perioperative Period of Umbilical Vein Catheterization

目的:探讨循证护理在新生儿脐静脉置管术(UVC)围术期中的应用效果。方法:回顾性分析2021年1月—2022年10月于鄂州市妇幼保健院接受UVC的78例新生儿的临床资料,根据护理方法不同分为两组,各39例。常规组实施常规护理干预,循证组实施循证护理干预。比较两组围术期指标、非计划拔管发生情况、并发症发生情况及家长满意度。结果:循证组UVC穿刺耗时、开始喂养时间短于常规组,脐静脉导管留置时间长于常规组,差异有统计学意义(P<0.001)。循证组非计划性拔管总发生率低于常规组,差异有统计学意义(P=0.048)。循证组并发症总发生率低于常规组,差异有统计学意义(P=0.042)。循证组家长对护士专业水平、技能熟练度、服务态度、风险防范及健康教育的评分高于常规组,差异有统计学意义(P<0.001)。结论:循证护理在新生儿UVC围术期中的应用效果显著,可缩短穿刺耗时与开始喂养时间,延长脐静脉导管留置时间,降低非计划性拔管及并发症发生率,提高家长满意度。

Objective:To explore the application effect of evidence-based nursing in neonates during perioperative period of umbilical vein catheterization(UVC).Methods:The clinical data of 78 newborns who underwent UVC in Ezhou Maternal and Child Health Hospital from January 2021 to October 2022 were retrospectively analyzed.According to the nursing methods,these newborns were divided into two groups,with 39 cases in each group.The routine group implemented routine nursing intervention,and the evidence-based group implemented evidence-based nursing intervention.The perioperative indicators,the incidence of unplanned extubation,the incidence of complications and the satisfaction of parents were compared between the two groups.Results:The time of UVC puncture and the time of starting feeding in the evidence-based group were shorter than those in the routine group,and the duration of indwelling umbilical vein catheter in the evidence-based group was longer than that in the routine group,and the difference was statistically significant(P<0.001).The total incidence of unplanned extubation in the evidence-based group was lower than that in the routine group,and the difference was statistically significant(P=0.048).The total incidence of complications in the evidence-based group was lower than that in the routine group,and the difference was statistically significant(P=0.042).The scores of parents on nurses'professional level,skill proficiency,service attitude,risk prevention and health education in the evidence-based group were higher than those in the routine group,and the difference was statistically significant(P<0.001).Conclusion:Evidence-based nursing has a significant application effect in neonates during perioperative period of UVC,and can shorten the time of UVC puncture and the time of starting feeding,prolong duration of indwelling umbilical vein catheter,reduce the incidence of unplanned extubation and complications,and increase the satisfaction of parents.
