RPH-4联合消痔灵注射治疗Ⅰ~Ⅲ期内痔的临床疗效

Clinical Efficacy of RPH-4 combined with Xiaozhiling Injection in the Treatment of StageⅠ~ⅢDegree Internal hemorrhoids

目的:观察自动弹力线痔套扎器(RPH-4)联合消痔灵注射治疗Ⅰ~Ⅲ期内痔的临床疗效。方法:选取2021年10月—2022年10月于安徽中医药大学第一附属医院肛肠科和上海中医药大学附属曙光医院安徽医院肛肠科Ⅰ~Ⅲ期内痔患者60例,随机分为观察组和对照组,各30例。观察组采用RPH-4联合消痔灵注射,对照组采用内痔结扎术治疗,比较2组临床疗效、围手术期相关情况(手术时间、术中出血、术后住院时间)、术后并发症(术后首次排便疼痛、术后出血、肛门坠胀),并比较术后半年复发率。结果:2组患者的一般资料、有效率及手术时间比较,差异均无统计学意义。观察组的术中出血量少于对照组,术后住院时间短于对照组,术后首次排便疼痛评分、术后出血评分及肛门坠胀评分均低于对照组,观察组的术后半年复发率低于对照组,差异均有统计学意义。结论:RPH-4联合消痔灵注射治疗Ⅰ~Ⅲ期内痔疗效确切,具疼痛轻、出血少、住院时间短、复发率低等优点,值得推广。

Objective:To observe the clinical efficacy of automatic elastic hemorrhoid ligation device(RPH-4)combined with Xiaozhiling injection in the treatment of stage I to III internal hemorrhoids.Methods:60 patients with stage I to III internal hemorrhoids from the Anorectal Department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine and Shuguang Hospital,Anhui Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from October 2021 to October 2022 were selected and randomly divided into observation group and Control group,30 cases each.The observation group was treated with RPH-4 combined with Xiaozhiling injection,and the control group was treated with internal hemorrhoid ligation.The clinical efficacy,perioperative related conditions(operative time,intraoperative bleeding,postoperative hospitalization time),and postoperative complications(Pain during the first defecation after surgery,postoperative bleeding,and anal swelling),and the recurrence rate six months after surgery was compared.Results:There was no statistically significant difference in the general information,effective rate and operation time between the two groups of patients.The intraoperative bleeding volume of the observation group was less than that of the control group,and the postoperative hospitalization time was shorter than that of the control group.The first postoperative defecation pain score,postoperative bleeding score,and anal swelling score were all lower than those of the control group.The observation group had a recurrence rate six months after surgery.The rate was lower than that of the control group,and the differences were statistically significant.Conclusion:RPH-4 combined with Xiaozhiling injection is effective in treating stage I to III hemorrhoids.It has the advantages of mild pain,less bleeding,short hospitalization time,and low recurrence rate,and is worthy of promotion.
