酒依赖患者大脑小世界网络属性的变化分析

Analysis of Changes in Brain Small⁃World Network Properties in Patients with Alcohol Dependence

摘要 目的探讨酒依赖患者静息态脑功能网络拓扑属性改变及其与认知功能和临床表现的关系。方法对53例酒依赖患者(AD组)和68例年龄、受教育年限相匹配的健康成年人(HC组)行静息态功能磁共振成像(rs-fMRI)扫描。采用图论构建大脑功能网络模型,比较两组小世界网络特征和脑区节点属性,分析脑网络变化与宾夕法尼亚酒精渴求量表(PACS)、贝克焦虑量表(BAI)、简易精神状态量表(MMSE)、数字广度测试(DST)的相关性。结果两组脑网络均呈现小世界网络特征(γ>1,λ≈1,σ>1)。两组的γ和σ值差异无统计学意义(P>0.05);在0.20~0.40稀疏度范围内,酒依赖组λ值显著低于健康对照组(P<0.05)。酒依赖组较健康对照组左侧岛叶及左侧杏仁核的度中心性升高,左侧颞横回的中介中心性升高,左侧壳核的中介中心性降低(P值均<0.05)。左侧岛叶与DST得分呈正相关(r=0.273,P=0.048),左侧杏仁核与PACS(r=0.322,P=0.019)、BAI(r=0.293,P=0.033)分数呈正相关。结论酒依赖患者脑功能网络的信息传输能力受损,图论脑网络分析可作为酒依赖患者大脑功能异常的一项影像学指标。 Objective To investigate the changes in topological properties of resting⁃state functional brain networks in alcohol⁃dependent patients and their relationship with cognitive function and clinical performance.Methods Resting⁃state functional magnetic resonance imaging scans were performed on 53 alcohol⁃dependent patients(AD group)and 68 age and education⁃matched healthy controls(HC group).The functional brain network model was constructed using graph theory to compare the small⁃world network characteristics and nodal properties of brain regions in the two groups,and to analyses the correlation between brain network changes and the Penn Alcohol Craving Scale(PACS),the Beck Anxiety Inventory(BAI),the Mini⁃Mental State Examination(MMSE),and the Digital Span Test(DST).Results The brain networks of both groups had small⁃world network characteristics(γ>1,λ≈1,σ>1).The differences inγandσbetween the two groups were not statistically significant(P>0.05).In the sparsity range of 0.20 to 0.40,λwas reduced in the alcohol⁃de⁃pendent group compared with the healthy control group,and the differences were statistically significant(P<0.05).The al⁃cohol⁃dependent group had higher degree centrality in the left insula and left amygdala,higher betweenness centrality in the left temporal transverse gyrus,and lower betweenness centrality in the left putamen compared with the healthy control group(all P<0.05).The left insula was positively correlated with DST scores(r=0.273,P=0.048),and the left amygdala was positively correlated with PACS(r=0.322,P=0.019)and BAI(r=0.293,P=0.033)scores.Conclusion The infor⁃mation transmission capacity of functional brain networks in alcohol⁃dependent patients is impaired,and graph theory analy⁃sis of brain network can be used as an imaging indicator of the abnormal brain function in patients with alcohol dependence.
