Roy适应模式指导的社区卫生服务属地管理对糖尿病前期患者的干预效果分析

Analysis of intervention effect of Roy adaptation model-guided community health service territorial management for patients with prediabetes mellitus

目的探讨糖尿病前期患者采用Roy适应模式指导的社区卫生服务属地管理的干预效果。方法选择2021年1月至6月于社区体检筛查及常规门诊中确诊的糖尿病前期患者120例,随机分为对照组(60例)和试验组(60例),对照组患者接受社区常规健康管理措施,试验组在对照组基础上给予Roy适应模式指导的社区卫生服务属地管理干预治疗。对比2组患者生活质量(健康状况调查问卷(SF)-36评分)和血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA_(1)c)水平]。结果干预前,2组间SF-36各维度评分差异均无统计学意义(P>0.05),干预12个月后,2组生理功能、生理职能、躯体疼痛、总体健康、活力、社会功能、情感职能和精神健康评分均高于同组干预前(P<0.05),且试验组高于同期对照组(P<0.05);干预前,2组间FPG、2 hPG、HbA_(1)c水平差异均无统计学意义(P>0.05),干预12个月后,2组FPG、2 hPG、HbA_(1)c水平均低于同组干预前(P均<0.05),且试验组低于同期对照组(P<0.05)。结论对糖尿病前期患者实施Roy适应模式指导的社区卫生服务属地管理干预,可明显改善患者血糖水平,提高生活质量。

Objective To explore the intervention effect of Roy adaptation model-guided community health service territorial management for patients with prediabetes mellitus.Methods A total of 120 patients with prediabetes mellitus diagnosed in community physical screening and routine outpatient services from January to June 2021 were randomly grouped into the control group(n=60)and the test group(n=60).The quality of life(SF-36 score)and blood glucose level[fasting blood glucose(FPG),2-hour postprandial blood glucose(2 hPG),hemoglobin A_(1)c(HbA_(1)c)]of the two groups were compared.Results Before the intervention,there was no statistical difference in SF-36 scores in each dimension between the two groups(P>0.05).After 12 months of intervention,the scores of physiological function,physiological function,body pain,overall health,vitality,social function,emotional function and mental health of the two groups were obviously higher than those before intervention(P<0.05),these indexes in the test group were obviously higher than those in the control group(P<0.05).Before the intervention,there was no statistical difference in the levels of FPG,2 hPG and HbA_(1)c between the two groups(P>0.05).After 12 months of intervention,the levels of FPG,2 hPG and HbA_(1)c in the two groups were obviously lower than those before intervention(P<0.05),and the three factors in the test group were obviously lower than those in the control group(P<0.05).Conclusion The implementation of Roy adaptation model-guided community health service territorial management intervention for pre-diabetes mellitus patients can obviously improve the blood sugar level of patients and improve their quality of life.
