头颈部神经源性肿瘤145例回顾性分析

A retrospective analysis of 145 cases with neurogenic tumors in head and neck region

目的总结头颈部神经源性肿瘤的诊疗经验。方法回顾性分析我院于2001年1月~2021年7月收治并经病理确诊的头颈部神经源性肿瘤145例,总结其临床病理特点及诊疗体会。结果本组病例中,包括神经鞘瘤99例,恶性周围神经鞘瘤5例,神经纤维瘤21例,副神经节瘤18例,节细胞神经母细胞瘤1例,节细胞神经瘤1例。其中111例以局部无痛性肿块为主要临床症状,96例肿瘤位于颈部。本组患者中144例接受了手术治疗,术后并发症主要为血管神经损伤。结论头颈部神经源性肿瘤多见青年、中年群体,神经鞘来源者最常见。手术完整切除是主要治疗手段,术中应尽量避免损伤重要的血管神经,恶性者需辅助放化疗。

Objective To sum up our experience with diagnosis and treatment on neurogenic tumors in the head and neck region.Methods A retrospective analysis was made on 145 cases with neurogenic tumors pathologically confirmed in the region of head and neck,treated in our hospital from January 2001 to July 2021,to sum up their clinical pathological characteristics and our experience with diagnosis and treatment on such a kind of conditions.Results In this group of cases,99 cases were diagnosed as schwannoma,5 cases as malignant peripheral nerve sheath tumor,21 as neurofibroma,18 as paraganglioma,1 as ganglioneuroblastoma,and 1 as ganglioneuroma.Among them,local painless mass was complained as the main clinical symptom in 111 cases,while 96 cases were found with their tumors located in the region of neck.In this group of patients,144 received surgical treatment,with the main post-operative complications being injury to blood vessels and/or nerves.Conclusion Neurogenic tumors in the region of head and neck are more frequently found in the population of younger and mid-aged persons,with neurosheath derived tumors being the commonest seen ones in clinic practice.Complete resection should be the widely used and most effective way to treat this kind of neurogenic tumors in the head and neck region.Attention must be paid to the operation being to avoid any injuries to the important vessels and nerves as far as possible.In case of malignant tumors,needed are adjuvant radiotherapy and/or chemotherapy.
