改良针刀术式治疗肩胛背神经卡压综合征的临床研究

Clinical Study of Modified Acupotomy in Treating Dorsal ScapularNerve Entrapment Syndrome

目的:观察改良针刀术式治疗肩胛背神经卡压综合征(DsnES)患者的临床疗效。方法:以符合纳入标准的60例DsnES患者为研究对象,随机分成两组。观察组以改良针刀术式治疗,对照组以痛点注射治疗。观察并记录各组患者治疗前后及随访时的VAS、NDI及SF-36评分,比较临床疗效。结果:观察组患者临床总有效率90.0%(27/30)和愈显率60%(18/30)均高于对照组,差异具有统计学意义(P<0.05)。两组患者疗程结束后及随访时VAS、NDI及SF-36评分均较治疗前明显改善,且观察组患者随访优势更明显,差异有统计学意义(P<0.05)。结论:改良针刀术式和痛点注射治疗均能改善DsnES患者症状,在短期疗效上,两种方式缓解患者疼痛、改善患者肩颈功能提高其生活质量水平相当;在长期疗效上,针刀治疗改善各项评分的效果更明显,随访时更有优势,改良针刀术式治疗本病具有更高的临床有效性。

Objective:To observe the clinical effect of modified acupotomy in treatment of dorsal scapular nerve entrapment syndrome(DsnES).Methods:A total of 60 patients with DsnES who met the inclusion criteria were randomly divided into the observation group and the control group.The observation group was treated with modified acupotomy,whereas the control group was treated with pain point injection.The scores of VAS,NDI and SF-36 were observed and recorded before the treatment,after the treatment and during the follow-up in the two groups.The clinical efficacy was also compared between the two groups.Results:The total effective rate was 90%(27/30)and the remarkably effective rate was 60%(18/30)in the observation group,which were significantly higher than 66.7%(20/30)and 30%(9/30)in the control group(P<0.05).The scores of VAS,NDI and SF-36 were significantly improved after the treatment and during the follow-up compared with those before the treatment in the two groups;of which the improvements were more significantly in the observation group(P<0.05).Conclusion:Both modified acupotomy and pain point injection can improve the symptoms of patients with DsnES.In terms of short-term efficacy,the two methods have similar levels of pain relief,shoulder and neck function improvement and quality of life improvement.In terms of long-term efficacy,acupotomy treatment has more obvious effects on improving each score,and has more advantages in the follow-up.The modified acupotomy method has a higher clinical effectiveness in treatment of DsnES.
