MRI多模态成像与CT图像融合技术对脑胶质瘤术后放疗患者放射剂量定制的影响

Influence of MRI Multimodal Imaging and CT Image Fusion Technology on Radiation Dose Customization in Patients with Glioma after Radiotherapy

摘要 目的:探究核磁共振(MRI)多模态成像与电子计算机断层扫描(CT)图像融合技术对脑胶质瘤术后放疗患者放射剂量定制的影响。方法:选取2018.9-2021.12期间于我院就诊的60例脑胶质瘤患者,将其分为CT组和MRI-CT组,每组各30例。CT组行CT检查后进行靶区勾画及计算放射剂量,而MRI-CT组则在分别进行MRI和CT检查后进行图像融合,根据融合图像勾画靶区并定制放射剂量。比较两组靶区体积[肿瘤靶区(GTV)、临床靶区(CTV)]、危及器官(左/右晶状体、脑干、海马区)OARs体积及放射剂量Dmax差异。结果:MRI-CT组与CT组相比较,其GTV、CTV、海马区OARs体积、脑干/海马区Dmax更低,差异有统计学意义(P<0.05),其余指标则无明显差异(P>0.05)。结论:MRI-CT能更精准的勾画靶区位置,合理规划放疗剂量,有利于脑胶质瘤放疗计划的制定。 Objective:To investigate the effect of multimodal magnetic resonance imaging(MRI)and computed tomography(CT)image fusion technology on radiation dose customization in patients with glioma undergoing postoperative radiotherapy.Methods:A total of 60 glioma patients who were treated in our hospital from September 2018 to December 2020 were selected and divided into CT group and MRI-CT group,with 30 cases in each group.In the CT group,the target volume was delineated and the radiation dose was calculated after CT examination.In the MRI-CT group,image fusion was performed after MRI and CT examination respectively,and the target volume was delineated and the radiation dose was customized according to the fusion image.The target volume[tumor target volume(GTV),clinical target volume(CTV)],organ at risk(left/right lens,brainstem,hippocampus)OARs volume and radiation dose Dmax were compared between the two groups.Results:Compared with the CT group,the MRI-CT group had lower GTV,CTV,OARs volume in the hippocampus,and Dmax in the brainstem/hippocampus,and the difference was statistically significant(P<0.05),while the other indicators had no significant difference(P>0.05).Conclusion:MRI-CT can more accurately delineate the location of the target area and reasonably plan the radiation dose,which is beneficial to the formulation of the radiation therapy plan for glioma.
