MRI诊断肝脏囊性占位性病变的价值

Diagnostic Value of MRI in Hepatic Cystic Lesions

目的:评价MRI诊断肝脏囊性占位性病变的价值。方法:选择2021年5月至2022年12月间本院收治的196例疑似肝脏囊性占位性病变患者,都接受MRI、螺旋CT检查,以病理检查结果为标准,比较MRI、CT的诊断效能,并且评价核磁共振ADC值定量鉴别各种肝脏囊性占位性病变的效果。结果:病理检查结果表明,187例为肝脏囊性占位性病变。MRI与螺旋CT诊断肝脏囊性占位性病变的准确度、特异度、敏感度均无显著差异(P>0.05)。螺旋CT鉴别炎症性囊性病变的符合率为86.27%(44/51),MRI为98.04%(50/51),显著高于螺旋CT(P<0.05);两种方法鉴别肝囊肿、肝血管瘤、肝肿瘤囊变或坏死的符合率则无显著差异(P>0.05)。4种肝脏囊性占位性病变的囊区ADC值、实区ADC值、综合ADC值均有统计学差异(P<0.05)。结论:MRI可以准确诊断肝脏囊性占位性病变,并且有效区分肝囊肿、肝脓肿、肝血管瘤、肝肿瘤囊变或坏死,具有较高的应用价值。

Objective:To evaluate the value of MRI in the diagnosis of hepatic cystic space occupying lesions.Methods:From May 2021 to December 2022,196 patients with suspected hepatic cystic space-occupying lesions in our hospital were examined by MRI and spiral CT,and the diagnostic efficacy of MRI and CT was compared based on pathological examination results,and the effect of quantitative identification of various hepatic cystic space-occupying lesions by ADC value of nuclear magnetic resonance was evaluated.Results:Pathological examination showed that 187 cases were hepatic cystic space occupying lesions:There was no significant difference in accuracy,specificity and sensitivity between MRI and spiral CT in diagnosing hepatic cystic space-occupying lesions(P>0.05).The coincidence rate of spiral CT in diferentiating inflammatory cystic lesions was 86.27%(44/51),and MRI was 98.04%(50/51),which was significantly higher than that of spiral CT(P<0.05).There was no significant difference between the two methods in differentiating hepatic cyst,hepatic hemangioma and cystic degeneration or necrosis of hepatic tumor(P>0.05).There were statistical differences in the ADC values of cystic area,solid area and comprehensive ADC values of the four kinds of hepatic cystic space occupying lesions(P<0.05).Conclusion:MRI can accurately diagnose hepatic cystic space-occupying lesions,and effectively distinguish hepatic cysts,hepatic abscesses,hepatic hemangioma and hepatic tumor cystic degeneration or necrosis,which has high application value.
