全肿瘤表观弥散系数直方图在腮腺分泌性癌与腺泡细胞癌中的鉴别价值

The Value of Whole-tumor Apparent Diffusion Coefficient Histogram in Differentiating Secretory Carcinoma from Acinic Cell Carcinoma of Parotid Gland

目的:探讨全肿瘤表观弥散系数(ADC)直方图在鉴别腮腺分泌性癌(SC)与腺泡细胞癌(AciCC)中的应用价值。方法:回顾性分析6例SC和14例AciCC的MRI资料。采用全肿瘤ADC直方图分析方法,比较两组肿瘤ADC直方图参数的差异,绘制受试者工作特征(ROC)曲线分析具有显著差异的参数以明确鉴别SC和AciCC的最佳ADC直方图参数。结果:两组肿瘤ADC的平均值,最大值,第5、25、50、75、95百分位数值,偏度的差异具有统计学意义(P<0.05),其中第95百分位数值的曲线下面积(AUC)最大,AUC为0.917,灵敏度为100.00%,特异度78.57%,准确度85.00%。结论:全肿瘤ADC直方图能够有效地鉴别腮腺SC和AciCC,并以第95百分位数值的鉴别诊断效能最高。

Purpose:To explore the application value of whole-tumor apparent diffusion coefficient(ADC)histogram in distinguishing secretory carcinoma(SC)from acinic cell carcinoma(AciCC)of the parotid gland.Methods:A retrospective analysis of MRI data was carried out on six SCs and 14 AciCCs.The whole-tumor ADC histogram analysis was utilized to compare the differences in ADC parameters between the two groups.The ROC curve was used to analyze the parameters with significant differences so as to clarify the optimal ADC histogram parameter for identifying SC and AciCC.Results:There were significant differences in mean,maximum,5th-,25th-,50th-,75th-,95th percentile values,and skewness derived from the ADC histogram between the two groups(P<0.05).The 95th percentile had the highest area under the curve(AUC)of 0.917,with the sensitivity of 100.00%,specificity of 78.57%,and accuracy of 85.00%.Conclusion:The whole-tumor ADC histogram could be used to effectively differentiate SC from AciCC of the parotid gland,and the 95th percentile with the highest differential diagnostic efficacy.
