摘要
目的探讨肝细胞癌(HCC)超声造影特征及其与患者血浆miR-30a水平的关系。方法选择2019年1月至2021年8月在新疆医科大学第二附属医院接受手术治疗的100例HCC患者(HCC组),其中男性74例,女性26例;年龄32~78岁,平均年龄58.24岁;肿瘤数量,1个67例,≥2个23例;直径3.1 cm(标准差0.23 cm)。同期纳入50例健康人群(对照组),其中男性35例,女性15例;年龄30~71岁,平均年龄56.24岁。对HCC及癌旁组织行超声造影检查。逆转录聚合酶链式反应(RT-PCR)检测两组血浆miR-30a水平。分析超声造影参数(始增时间、达峰时间、峰值加速时间、始增速度及增强速率)与miR-30a水平的关系。结果HCC的始增时间、达峰时间、峰值加速时间均低于癌旁组织[(9.93±2.01)s vs(19.84±1.23)s、(22.73±3.82)s vs(37.02±4.18)s、(12.43±1.44)s vs(23.65±3.17)s。均P<0.05],始增速度和增强速率均高于癌旁组织[(9.21±0.84)dB vs(6.14±0.61)dB、(2.33±0.11)dB/s vs(1.27±0.15)dB/s。均P<0.05]。HCC患者血浆miR-30a水平(0.12±0.03)低于对照组(0.28±0.04)(P=0.000)。miR-30a低表达组患者始增时间、达峰时间、峰值加速时间均低于miR-30a高表达组[(7.84±0.83)s vs(11.83±1.02)s、(17.37±3.11)s vs(24.08±2.16)s、(9.04±0.13)s vs(14.03±1.02)s。均P<0.05],始增速度和增强速率均高于miR-30a高表达组[(10.84±1.03)dB vs(7.22±0.36)dB、(2.98±0.09)dB/s vs(1.87±0.11)dB/s。均P<0.05]。结论超声造影检查可有效显示HCC影像学特征,其影像学特征与患者血浆miR-30a水平存在一定相关性。
Objective To investigate the characteristics of contrast-enhanced ultrasound(CEUS)in patients with hepatocellular carcinoma(HCC)and its correlation with plasma miR-30a level.Methods From January 2019 to August 2021,a total of 100 patients with HCC who underwent surgical treatment were enrolled,which included 74 males and 26 females,aged 32-78 years old with mean age of 58.24 years old;number of tumors was 1 tumor in 67 cases and≥2 in 23 cases,and diameter was 3.1 cm(standard deviation 0.23 cm).In the same period,50 healthy persons were enrolled as control group,which included 35 males and 15 females,aged 30-71 years old with mean age of 56.24 years old.The CEUS was performed on HCC and adjacent normal liver tissues.The plasma miR-30a level was detected by reverse transcription polymerase chain reaction(RTPCR).Correlation between CEUS parameters(initial increase time,peak time,peak acceleration time,initial increase speed and enhancement rate)and plasma miR-30a level was analyzed.Results The initial increase time,peak time and peak acceleration time of HCC were significantly lower than those of adjacent normal liver tissues[(9.93±2.01)seconds vs(19.84±1.23)seconds,(22.73±3.82)seconds vs(37.02±4.18)seconds,(12.43±1.44)seconds vs(23.65±3.17)seconds.P<0.05].The initial growth rate and enhancement rate of HCC were significantly higher than those of normal liver tissues[(9.21±0.84)dB vs(6.14±0.61)dB,(2.33±0.11)dB/s vs(1.27±0.15)dB/s.P<0.05].The level of plasma miR-30a in HCC group(0.12±0.03)was significantly lower than that in control group(0.28±0.04)(P=0.000).The initial increase time,peak time and peak acceleration time of patients in miR-30a low expression group were lower than those in miR-30a high expression group[(7.84±0.83)seconds vs(11.83±1.02)seconds,(17.37±3.11)seconds vs(24.08±2.16)seconds,(9.04±0.13)seconds vs(14.03±1.02)seconds.P<0.05],and the initial increase speed and enhancement rate in miR-30a low expression group were higher than those in miR-30a high expression group[(10.84±1.03)dB vs(7.22±0.36)dB,(2.98±0.09)dB/s vs(1.87±0.11)dB/s.P<0.05].Conclusion It is demonstrated that CEUS can effectively display the imaging features of HCC,and its imaging features are related to the level of plasma miR-30a.
