厦门三师共管模式在高血压合并糖尿病患者中的作用

The Role of Xiamen Three Division Condominium Model In Patients with Hypertension and Diabetes Mellitus

目的 探讨厦门三师共管模式在高血压合并糖尿病患者中的作用。方法 选取2021年7~12月于我中心医治的高血压合并糖尿病患者76例,以随机数字表法对全部受试对象进行组别划分,将其分为对照组(常规慢性病管理服务,n=38)与管理组(在对照组的基础上采用家庭医生签约“三师共管”服务模式,n=38),对比两组患者血压及血糖变化水平、患者治疗依从性。结果 两组管理前血糖指标水平对比无较大差别(P> 0.05);管理后,管理组空腹血糖、糖化血红蛋白水平显著低于对照组(P <0.05)。两组管理前血压指标水平对比无较大差别(P> 0.05);管理组管理后收缩压、舒张压水平显著低于对照组(P <0.05)。管理组治疗依从率92.11%(35/38)显著高于对照组73.68%(28/38)(P <0.05)。结论 厦门三师共管模式可改善高血压合并糖尿病患者的血糖及血压水平,并有效提高患者治疗依从性。

Objective To investigate the effect of three division condominium mode in patients with hypertension and diabetes mellitus.Methods A total of 76 patients with hypertension and diabetes mellitus,who were treated in our center from July to December in the 20212021,were divided into two groups by random number table,they were divided into the control group(routine chronic disease management service,n=38)and the management group(on the basis of the control group,family doctors signed the“Three division co-management”service model,n=38),the changes of blood pressure and blood glucose and the compliance of patients were compared between the two groups.Results There was no significant difference in blood glucose index levels between the two groups before management(P>0.05);After management,the levels of fasting blood glucose and glycated hemoglobin in the management group were significantly lower than those in the control group(P<0.05).There was no significant difference in blood pressure index levels between the two groups before management(P>0.05);The levels of systolic blood pressure and diastolic blood pressure in the management group after management were significantly lower than those in the control group(P<0.05).The treatment compliance rate of 92.11%(35/38)in the management group was significantly higher than 73.68%(28/38)in the control group(P<0.05).Conclusions Xiamen three division co management mode can improve the blood glucose and blood pressure levels of patients with hypertension and diabetes mellitus,and effectively improve the treatment compliance of patients.
