FOXA1蛋白和Wnt/β-catenin信号通路相关蛋白在卵巢癌组织中的表达及临床意义

摘要 目的分析上皮性卵巢癌(EOC)组织中叉头框蛋白A1(FOXA1)与Wnt/β-catenin信号通路相关因子β-联环素(β-catenin)的表达情况,并探讨其临床意义。方法收集60例EOC癌组织标本(EOC组),42例上皮性卵巢良性肿瘤组织标本(良性组),以及30例正常卵巢上皮组织标本(对照组),采用免疫组织化学法检测标本中FOXA1与β-catenin蛋白表达;分析FOXA1与β-catenin蛋白表达与EOC临床病理特征、预后的关系。结果EOC组患者癌组织中FOXA1阳性表达率明显高于良性组和对照组,β-catenin阳性表达率低于良性组和对照组,差异均有统计学意义(χ^(2)分别=19.59、21.37、11.78、25.70,P均<0.05);EOC患者癌组织FOXA1阳性表达率与FIGO分期、肿瘤大小、淋巴结转移相关(χ^(2)分别=7.25、7.55、8.16,P均<0.05);β-catenin阳性表达率与FIGO分期、淋巴结转移、病理分级相关(χ^(2)分别=27.80、5.71、6.17,P均<0.05);FOXA1阳性组总生存率低于FOXA1阴性组,β-catenin阳性组总生存率高于β-catenin阴性组,差异均有统计学意义(χ^(2)分别=5.98、4.75,P均<0.05);FIGO分期Ⅲ/Ⅳ期、合并淋巴结转移、病理分级G3、FOXA1阳性是EOC患者总生存率的危险因素,β-catenin阳性是保护因素(HR分别=2.39、2.13、1.51、3.40、0.68,P均<0.05)。结论EOC患者癌组织中FOXA1阳性表达、β-catenin阴性表达可能预示着卵巢癌分期晚、存在淋巴结转移以及预后不良。 Objective To analyze the expression of forkhead box protein A1(FOXA1)and Wnt/β-catenin signaling pathway-related factor(β-catenin)in epithelial ovarian cancer(EOC)tissues,and to explore their clinical significance.Methods Sixty EOC tissues(EOC group),42 epithelial ovarian benign tumor tissues(benign group)and 30 normal epithelial ovarian tissues(control group)were collected.The expression of FOXA1 andβ-catenin proteins in the above tissue samples was detected by immunohistochemistry.The relationship between the expression of FOXA1 andβ-catenin protein and the clinicopathological features,prognosis of EOC patients was analyzed.Results The positive expression rate of FOXA1 in EOC group was significantly higher than that in benign group and normal control group,while positive expression rate ofβ-catenin was lower than that in benign group and normal control group(χ2=19.59,21.37,11.78,25.70,P<0.05).The positive expression rate of FOXA1 in cancer tissues of EOC patients was related with FIGO staging,tumor size and lymph node metastasis(χ2=7.25,7.55,8.16,P<0.05).The positive expression rate ofβ-catenin was relat⁃ed with FIGO staging,lymph node metastasis and pathological grading(χ2=27.80,5.71,6.17,P<0.05).The overall sur⁃vival(OS)status in FOXA1 positive group was lower than that in FOXA1 negative group,which was worse inβ-catenin positive group thanβ-catenin negative group(χ2=5.98,4.75,P<0.05).The results showed that FIGO stageⅢ/Ⅳ,lymph node metastasis,pathological grading G3,positive FOXA1 were independent risk factors of OS in EOC patients,and posi⁃tiveβ-catenin was protective fartor(HR=2.39,2.13,1.51,3.40,0.68,P<0.05).Conclusion The positive expression of FOXA1 and negative expression ofβ-catenin in cancer tissues of EOC patients may indicate late ovarian cancer stage,lymph node metastasis and poor prognosis.
