比较牙周辅助加速成骨正畸与常规正畸治疗对患者牙周健康的影响

Comparison of the Effects of Periodontally Accelerated Osteogenic Orthodontics and Conventional Orthodontic Treatment on Periodontal Health of Orthodontic Patients

目的比较牙周辅助加速成骨正畸与常规正畸治疗对成人患者牙周健康状况的影响。方法选取我院2020年10月~2021年10月收治的90例接受正畸治疗的患者作为研究对象,根据治疗方式不同分为实验组(n=34)和对照组(n=56),试验组患者接受牙周辅助加速成骨正畸治疗,对照组患者接受常规正畸治疗。对比两组患者治疗前后牙周健康指标【包括角化龈宽度、菌斑指数(PLI)、临床附着水平(AL)】、龈沟液炎性因子水平【白介素-6(IL-6)、肿瘤坏死细胞-α(TNF-α)】及治疗期间不良反应发生率。结果正畸治疗结束后,实验组角化龈宽度、PLI、AL水平均显著低于对照组(P<0.05);与治疗前相比,两组龈沟液中炎性因子水平均下降,且实验组IL-6、TNF-α表达水平明显低于对照组(P<0.05);治疗过程中,实验组患者不良反应发生率5.55%(2/34)显著低于对照组21.42%(12/56)(P<0.05)。结论相比常规正畸治疗,牙周辅助加速成骨正畸治疗能有效维护牙周组织的健康,降低治疗风险,有利于正畸治疗效果的长期稳定。

Objective To compare the effects of periodontally accelerated osteogenic orthodontics and conventional orthodontic treatment on periodontal health of patients undergoing orthodontics treatment.Methods Ninety patients underwent orthodontic treatment in our hospital from October 2020 to October 2021 were enrolled,and divided into two groups according to different treatment methods.Experimental group(n=34)received periodontally accelerated osteogenic orthodontics,while control group(n=56)received conventional orthodontic traction.Then the periodontal health indexes[keratinized gingival width,plaque index(PLI),clinical attachment level(CAL)],inflammatory factors in gingival crevicular fluid[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and the adverse reaction rate were compared between two groups.Results After orthodontics treatment,the keratinized gingival width,PLI and CAL were significantly lower in experimental group than in control group(P<0.05).After treatment,a decrease was observed in the levels of inflammation factors in gingival crevicular fluid,and the levels of IL-6 and TNF-αof experimental group were obviously lower than those of control group(P<0.05).The adverse reaction rate during the treatment was 5.55%(2/34)in experimental group,which was significantly lower than 21.42%(12/56)in control group,with statistical difference(P<0.05).Conclusion Compared with conventional orthodontic treatment,application of periodontally accelerated osteogenic orthodontics can effectively maintain periodontal health status,reduce the treatment risk and ensure the long-term stability of orthodontic treatment.
