振腹法对多囊卵巢综合征大鼠血糖、胰岛素水平及p38丝裂原活化蛋白激酶蛋白表达的影响

The effects of abdominal vibration therapy on blood glucose,insulin and p38 MAPK protein expression in rats with polycystic ovary syndrome

摘要 目的探讨振腹法治疗对多囊卵巢综合征(polycystic ovarian syndrome,PCOS)大鼠空腹血糖(Fasting plasma glucose,FPG)、血清胰岛素(fasting serum lisulin,FINS)、胰岛素抵抗指数(Homeostatic model assessment for insulin resistance,HOMA-IR)及卵巢p38丝裂原活化蛋白激酶(p38 mitogen-activated protein kinase,p38 MAPK)、磷酸化p38丝裂原活化蛋白激酶(p-p38 mitogen-activated protein kinase,p-p38 MAPK)蛋白表达与卵巢组织结构的影响,为振腹法的临床应用推广提供科学依据。方法采用芳香酶抑制剂(来曲唑)造模法建立PCOS大鼠模型,将40只雌性SD大鼠随机分为空白组、模型组、二甲双胍组、振腹组,每组10只,各组大鼠给予相应药物连续灌胃21天。用葡萄糖氧化酶法测定FPG水平、酶联免疫吸附测定法测定空腹FINS水平,并计算HOMA-IR,比较各组的大鼠卵巢形态结构的差异;用蛋白免疫印迹法测定各组大鼠卵巢组织p38 MAPK、p-p38 MAPK蛋白表达量。结果与空白组相比,模型组大鼠卵巢出现囊性卵泡,卵巢皮质间质细胞明显增生,颗粒细胞层数减少;FPG、FINS、HOMA-IR水平升高(P<0.05),p38 MAPK、p-p38 MAPK表达量升高(P<0.05)。与模型组相比,二甲双胍组、振腹组大鼠卵巢囊性卵泡减少,颗粒细胞层增厚,可见卵母细胞,可见少量黄体;FPG、FINS、HOMA-IR水平降低(P<0.05),p38 MAPK、p-p38 MAPK表达量降低(P<0.05)。与二甲双胍组相比,振腹组大鼠FPG、p38 MAPK、p-p38 MAPK水平均相近(P>0.05)。结论振腹法可以改善PCOS模型大鼠卵巢结构,降低PCOS卵巢组织p38 MAPK、p-p38 MAPK蛋白表达水平,对PCOS血糖、胰岛素有一定调节作用,对PCOS伴胰岛素抵抗能有一定治疗作用,且振腹法在降低FPG水平上作用与二甲双胍作用相近。 Objective To investigate the effects of abdominal vibration therapy on fasting plasma glucose(FPG),serum insulin(fins),insulin resistance index(HOMA-IR),and the protein expressions of ovarian p38 mitogen activated protein kinase(p38 MAPK),phospho-p38 mitogen activated protein kinase(p-p38 MAPK)and ovarian tissue architecture in polycystic ovary syndrome(PCOS)rats,to provide a scientific basis for the promotion of clinical application of abdominal vibration therapy method.Methods The rat model of PCOS was established by aromatase inhibitor(letrozole),and a total of 40 female SD rats were randomly divided into normal control,model,metformin,and abdominal vibration therapy groups(n=10),each group was given corresponding drugs by continuous gavage for 21 days,fasting plasma glucose(FPG)levels were measured by glucose oxidase assay,and fasting insulin(fins)levels were measured by enzyme-linked immunosorbent assay,and the insulin resistance index(HOMA-IR)was calculated to compare the morphological and structural differences in the ovaries of the rats in each group,and the protein expression levels of p38 MAPK,p-p38 MAPK in the ovaries of each group were determined by Western blot.Results Compared with the blank group,the ovaries of model rats exhibited cystic follicles,and the interstitial cells in the ovarian cortex were significantly increased,the number of granulosa cell layers was decreased,the levels of FPG,fins,HOMA-IR were increased(P<0.05),and the expressions of p38 MAPK,p-p38 MAPK were increased(P<0.05);compared with the model group,the ovaries of rats in the metformin group or the abdominal vibration therapy group showed fewer cystic follicles,thickened granulosa cell layer,visible oocytes,a few corpora lutea were observed,FPG,fins,HOMA-IR levels were decreased(P<0.05),and p38 MAPK,p-p38 MAPK expression was decreased(P<0.05);compared with the metformin group,the FPG levels and p38 MAPK,p-p38 MAPK expression of rats in the abdominal vibration therapy group were similar(P>0.05).Conclusion The abdominal vibration method can improve the ovarian structure of PCOS model rats,reduce the expression level of p38 MAPK and p-p38 MAPK protein in PCOS ovarian tissue,regulate the blood glucose and insulin of PCOS,and have a certain therapeutic effect on PCOS with insulin resistance.The abdominal vibration method has a similar effect to metformin in reducing the FPG level.
