半夏泻心汤加减治疗肝胃不和证反流性食管炎合并反酸临床研究

Clinical Study on Modified Banxia Xiexin Decoction for Reflux Esophagitis Complicated with Acid Reflux with Liver-Stomach Disharmony Syndrome

目的:观察半夏泻心汤加减治疗肝胃不和证反流性食管炎合并反酸的临床疗效及对血清胃泌素(GAS)、胃动素(MLT)水平的影响。方法:选取80例肝胃不和证反流性食管炎合并反酸患者,随机分为研究组和对照组各40例。对照组给予常规治疗,研究组在对照组基础上结合半夏泻心汤加减治疗。比较2组疗效,以及治疗前后中医证候评分及GAS、MLT水平。结果:研究组总有效率为92.5%,对照组为75.0%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组呃逆嗳气、吞酸烧心、胸胁胀满、情志抑郁等中医证候评分比较,差异无统计学意义(P>0.05);治疗后,2组上述各项中医证候评分均较治疗前降低(P<0.05),且研究组各项评分均低于对照组(P<0.05)。治疗前,2组GAS、MLT水平比较,差异无统计学意义(P>0.05);治疗后,2组GAS、MLT水平均较治疗前升高(P<0.05),且研究组GAS、MLT水平均高于对照组(P<0.05)。结论:半夏泻心汤加减治疗肝胃不和证反流性食管炎合并反酸疗效显著,有助于改善患者中医证候,增强胃肠运动功能。

Objective:To observe the clinical effect of modified Banxia Xiexin Decoction for reflux esophagitis complicated with acid reflux with liver-stomach disharmony syndrome and its effects on the levels of gastrin(GAS) and motilin(MLT) in serum.Methods:A total of 80 cases of patients with reflux esophagitis complicated with acid reflux with liver-stomach disharmony syndrome were selected and randomly divided into the study group and the control group,with 40 cases in each group.The traditional Chinese medicine(TCM) syndrome scores and the levels of GAS and MLT before and after treatment,as well as curative effects,were compared between the two groups.Results:The total effective rate was92.5% in the study group and 75.0% in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the TCM syndrome scores of hiccup and belching,acid swallowing and heartburn,fullness in the chest and hypochondrium,and depression between the two groups(P>0.05);after treatment,the above TCM syndrome scores in the two groups were decreased when compared with those before treatment(P<0.05),and each score in the study group was lower than that in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of the levels of GAS and MLT between the two groups(P>0.05);after treatment,the levels of GAS and MLT in the two groups were increased when compared with those before treatment(P<0.05),and the two levels in the study group were higher than those in the control group(P<0.05).Conclusion:Modified Banxia Xiexin Decoction has a significant curative effect in the treatment of reflux esophagitis complicated with acid reflux with the liver-stomach disharmony syndrome and is conduce to the improvement of TCM syndromes and enhance gastrointestinal motility.
