重度食管静脉曲张胃镜下密集套扎联合白蛋白针静脉滴注效果分析

Effect Analysis of Intensive Ligation Combined with Albumin Needle Intravenous Drip under Gastroscope for Severe Esophagealvarices

目的:研究胃镜下密集套扎联合白蛋白针静脉滴注治疗重度食管静脉曲张的效果。方法:选取2020年6月—2022年6月江西省宁都县人民医院消化内科收治的重度食管静脉曲张患者80例作为研究对象,按随机数字表法分为对组照与观察组,各40例。对照组采用螺旋形套扎术,观察组采用胃镜下密集套扎联合白蛋白针治疗。比较两组改善总有效率,静脉曲张完全消除率与平均套扎次数,72 h和2周内再出血率及并发症发生情况。结果:观察组患者总有效率高于对照组,差异有统计学意义(P=0.018);观察组第1次、第2次、第3次套扎术后,静脉曲张完全消除率高于对照组,平均套扎次数低于对照组,差异有统计学意义(P<0.05);两组72 h和2周内再出血率比较,差异无统计学意义(P>0.05);两组术后胸骨疼痛、发热、食管狭窄发生率比较,差异无统计学意义(P>0.05)。结论:重度食管静脉曲张胃镜下使用密集套扎联合白蛋白针静脉滴注可以加快食管静脉曲张治疗康复速度,减少胃镜下套扎治疗的次数,提高依从性。

Objective:To study the effect of gastroscopic variceal ligation combined with intravenous albumin infusion in the treatment of severe esophageal varices.Methods:80 patients with severe esophageal varices admitted to the Department of Gastroenterology of Ningdu County People's Hospital of Jiangxi Province from June 2020 to June 2022 were selected as the study subjects,and were divided into the control group control and the observation group according to the method of randomized numerical table,each with 40 cases.The control group was treated with spiral ligation while the observation group was treated with gastroscopic ligation combined with albumin infusion.The following indexes of patients were compared in two groups:the total effective rate and elimination rate of varicose veins,and operative amount of ligations,the re-bleeding rate in 72 h and 2 weeks and the occurrence of complications.Results:the total effective rate of patients in the observation group was higher than that of the control group,and the difference was statistically significant(P=0.018);the complete elimination rate of varicose veins in the observation group was higher than that of the control group after the 1st,2nd,and 3rd operations,and Ligation operations was lesser than that of the control group,and the difference was statistically significant(P<0.05);the comparison of re-bleeding rates in the two groups after 72 hours and within 2 weeks,and the difference was statistically not significant(P>0.05);the difference was not statistically significant when comparing the incidence of postoperative sternal pain,fever,and esophageal stricture between the two groups(P>0.05).Conclusion:Gastroscopic variceal ligation combined with intravenous albumin infusion for severe esophageal varices can accelerate the process of recovery,avoid unnecessary ligation operation,and improve patient compliance.
