针刺联合中药外敷治疗乳腺癌改良根治术后肩关节功能障碍的效果

Therapeutic Effect of Acupuncture Combined with Traditional Chinese Medicine Herbal External Therapy in Treating Shoulder Joint Dysfunction after Modified Radical Mastectomy in Breast Cancer

目的:探讨针刺联合中药外敷治疗乳腺癌改良根治术后肩关节功能障碍的临床效果。方法:选取2021年2月—2023年2月于中山大学孙逸仙纪念医院就诊的乳腺癌改良根治术后肩关节功能障碍患者120例作为研究对象,随机分为研究组和对照组,各60例。对照组给予中药外敷,研究组在此基础上给予针刺治疗。比较两组治疗前后患侧肩关节活动度、上肢功能[手臂动作调查测试(ARAT)评分]和生活质量[健康调查简表(SF-36)评分]。结果:治疗后,两组肩关节后伸、前屈、外展、内收活动度增大,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组ARAT评分较治疗前升高,研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组SF-36评分均较治疗前升高,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:针刺联合中药外敷治疗乳腺癌改良根治术后肩关节功能障碍可有效改善患者患侧肩关节活动度及上肢功能,进而提高其生活质量,疗效确切。

Objective:To investigate the therapeutic effect of acupuncture combined with TCM herbal external therapy in treating shoulder joint dysfunction after modified radical Mastectomy of breast cancer.Methods:120 patients with shoulder joint dysfunction after modified radical mastectomy attending Sun Yat-sen Memorial Hospital,Sun Yat-sen University from February 2021 to February 2023 were selected as the study subjects,and were randomly divided into the study group and the control group,60 cases each.The control group was given TCM herbal external therapy,and the study group was given acupuncture treatment on this basis.The indexes such as motilities of the affected shoulder joint,upper limb function[Arm Movement Investigation Test(ARAT)score]and quality of life[Short Form Health Survey(SF-36)score]were compared between the two groups before and after treatment.Results:After treatment,the shoulder joint posterior extension,anterior flexion,abduction,and adduction mobility increased in both groups,and the study group was higher than the control group,with statistically significant differences(P<0.05);after treatment,the ARAT scores of the two groups increased compared with the pre-treatment period,and the study group was higher than the control group,with statistically significant differences(P<0.05);after treatment,the SF-36 scores of the two groups were higher compared with the pre-treatment period,and the study group was higher than the control group,with statistically significant differences(P<0.05).Conclusion:Acupuncture combined with TCM herbal external therapy in treating shoulder dysfunction after modified radical surgery for breast cancer can effectively improve the mobility of the shoulder joint and the function of the upper limb of the affected side of the patients,and then improve their quality of life,and the therapeutic effect is accurate.
