女性生殖道感染诊断中阴道分泌物解脲支原体的检出及耐药情况分析

Analysis of Detection Rate of Ureaplasma Urealyticum in Vaginal Secretion in Female Reproductive Tract infections and Its Drug Resistance Situation

摘要 目的:分析女性生殖道感染诊断中阴道分泌物解脲支原体的检测率,并探讨解脲支原体的耐药情况。方法:选取2022年1—12月湖南省娄底市妇幼保健院收治的疑似生殖道感染的女性患者618例为研究对象。采集患者生殖道分泌物标本,进行培养鉴定和体外药敏试验,分析检测结果。结果:618例患者中共检出356例支原体感染阳性患者,检出率为57.61%;解脲支原体阳性检出率高于人型支原体阳性检出率、混合阳性检出率,混合阳性检出率高于人型支原体阳性检出率。21~30岁年龄段患者阳性检出率高于其他年龄段,差异有统计学意义(P=0.0001)。喹诺酮类药物中环丙沙星的耐药率最高,为92.22%;大环内酯类药物中红霉素的耐药率最高,为74.81%;四环素类药物中米诺环素的敏感性最高,为95.93%。结论:女性生殖道感染患者易发生解脲支原体感染,通过检测患者阴道分泌物解脲支原体,能为诊断女性生殖道感染提供指导,而发病年龄段主要为21~30岁女性。另外,女性生殖道解脲支原体存在严重的耐药现象,特别是对喹诺酮类药物。 Objective:To analyze the detection rate of Ureaplastma urealyticum(UU)in vaginal secretions in the diagnosis of female reproductive tract infections and to explore its drug resistance.Methods:618 female patients with suspected reproductive tract infections admitted to Loudi Maternal and Child Health Hospital in Hunan Province from January to December 2022 were selected for the study.Their vaginal secretions were collected for germiculture and drug sensitivity test.The test results were subsequently analyzed.Results:356 out of 618 patients were detected with positive UU.The detection rate was 57.61%;the UU detection rate was higher than that of Mycoplasma hominis(MH),while mixed positive detection rate,and the mixed positive detection rate was higher than those of MH.The UU detection rate of the patients in the age group of 21-30 years old was higher than those in other age groups,and the difference was statistically significant(P=0.0001).The drug resistance rate of ciprofloxacin was the highest among quinolone antibiotic with 92.22%;erythromycin was the highest among macrolides with 74.81%;and minocycline had the highest sensitivity among tetracyclines with 95.93%.Conclusion:UU infection is one of susceptible female genital tract infection.Microbiological test of UU in patients'vaginal secretions can provide guidance for clear diagnostic.The age group of 21 to 30 year old is the susceptible group.In addition,female genital tract UU infection is severely drug-resistant,especially to quinolones antibiotics.
