彩色多普勒超声对子宫腺肌病与子宫肌瘤的临床诊断与鉴别

Clinical Diagnosis and Differentiation of Adenomyopathy and Uterine Myoma by Color Doppler Ultrasonography

目的:探讨彩色多普勒超声对子宫腺肌病与子宫肌瘤的临床鉴别诊断价值。方法:选取2021年5月—2022年10月于贵州省人民医院收治的子宫腺肌病与子宫肌瘤患者100例作为研究对象,其中子宫腺肌病患者68例为子宫腺肌病组,子宫肌瘤患者32例为子宫肌瘤组。所有患者行经腹部和经阴道彩色多普勒超声检查,比较子宫腺肌病、子宫肌瘤血流参数及血流信号分级,两种超声检查满意度。结果:经阴道超声检查子宫腺肌病组血流参数收缩期血流、舒张期血流信号、搏动指数高于子宫肌瘤组,差异有统计学意义(P<0.05);两组血流参数指血流阻力指数水平比较,差异无统计学意义(P>0.05);经阴道超声检查总满意度高于经腹部超声检查,差异有统计学意义(P=0.001)。结论:彩色多普勒超声能够为子宫腺肌病与子宫肌瘤的临床诊断提供清晰图像,医生可根据超声检查的声像特征、血流信号科学判断患者的实际病情,经阴道彩色多普勒超声检查准确率更高,对子宫腺肌病与子宫肌瘤的特异性更高,有利于医生进行鉴别,提高诊断准确率。

Objective:To investigate the value of color Doppler ultrasound in the clinical differential diagnosis of adenomyosis and uterine fibroids.Methods:100 patients with either adenomyosis or uterine fibroids admitted to Guizhou Provincial People's Hospital from May 2021 to October 2022 were selected for the study,of which 68 patients with adenomyosis were in the adenomyosis group while another 32 patients with uterine fibroids in the uterine fibroids group.All patients underwent transabdominal and transvaginal color Doppler ultrasonography to compare the blood flow parameters and blood flow signal grading of uterine adenomyosis and uterine fibroids,and results of two types of ultrasonography were compared.Results:The blood flow parameters systolic blood flow,diastolic blood flow signal,and pulsatility index of transvaginal ultrasonography in the adenomyosis group were higher than those of the uterine fibroids group,and the difference was statistically significant(P<0.05);the levels of blood flow parameters referring to the resistance index of blood flow of the two groups were compared,and the differences were not statistically significant(P>0.05);the total satisfaction of transvaginal ultrasonography was higher than that of transabdominal ultrasonography,and the difference was statistically significant(P=0.001).Conclusion:Color Doppler ultrasound can provide clear images for the clinical diagnosis of uterine adenomyosis and uterine fibroids,and doctors can determine the actual condition of patients according to the acoustic features and blood flow signals of ultrasound,and transvaginal color Doppler ultrasonography has a higher accuracy and specificity for uterine adenomyosis and uterine fibroids,which is conducive to differential diagnosis and improves the diagnostic accuracy.
