钛夹在结肠息肉内镜下黏膜切除术中的应用效果

Application effect of titanium clamps in endoscopic mucosal resection of colon polyps

目的探讨钛夹在结肠息肉内镜下黏膜切除术中的应用效果。方法回顾性选取2019年1月至2022年1月山东颐养健康集团淄博医院消化内科收治的252例结肠息肉患者作为研究对象,均行内镜下黏膜切除术,根据术后是否常规应用钛夹闭合创面分为A组(n=127)和B组(n=125)。A组患者内镜黏膜切除术后常规应用钛夹闭合创面,B组患者单纯行内镜黏膜切除术治疗,比较两组患者腺瘤大小、分布差异,术后迟发性出血率,二次手术率,住院费用。结果两组患者腺瘤均一次性完整切除,两组患者腺瘤大小、分布比较,差异无统计学意义(P>0.05)。A组术后迟发性出血率、二次手术率均低于B组,差异有统计学意义(P<0.05)。两组患者住院费用比较,差异无统计学意义(P>0.05)。结论结肠息肉内镜下黏膜切除术后常规应用钛夹闭合创面,可有效降低术后迟发性出血率及二次手术率。

Objective To investigate the application effect of titanium clamps in endoscopic mucosal resection of colon polyps.Methods A total of 252 patients with colonic polyps admitted to the Department of Gastroenterology of Shandong Health Group Zibo Hospital from January 2019 to January 2022 were retrospectively selected as the research subjects.All patients underwent endoscopic mucosal resection,and they were divided into group A(n=127)and group B(n=125)according to whether titanium clamps were routinely applied to the wound after surgery.Patients in group A were treated with titanium clamps routinely after endoscopic mucosal resection,while patients in group B were treated with endoscopic mucosal resection alone.Differences in the size and distribution of adenomas,the rates of postoperative delayed bleeding,second operation rate,and hospitalization costs were compared between the two groups.Results The adenomas of two groups of patients were completely removed at one time,and there were no statistical significances in the size and distribution of adenomas between the two groups(P>0.05).The rates of postoperative delayed bleeding and second operation in group A were lower than those in group B,and the differences were statistically significant(P<0.05).There was no significant difference in hospitalization cost between the two groups(P>0.05).Conclusion The conventional application of titanium clamps on the wound after endoscopic mucosal resection of colon polyps can effectively reduce the rates of delayed postoperative bleeding and second operation.
