祛湿排浊汤治疗持续性痛风性膝关节炎疗效研究

Study on curative effect of Qushi Paizhuo decoction on persistent gouty knee arthritis

目的:分析祛湿排浊汤治疗持续性痛风性膝关节炎对膝关节活动度、疼痛的改善效果。方法:选择126例持续性痛风性膝关节炎患者进行回顾性研究,根据治疗方法的不同将其分为两组,每组63例。对照组给予非布司他、双氯芬酸钠等对症治疗,观察组在对照组基础上给予祛湿排浊汤治疗。比较两组临床疗效、膝关节屈曲活动度、视觉模拟自评量表(VAS)评分、关节功能(Leqensne)评分、滑膜厚度、血沉(ESR)、肿瘤坏死因子-α(TNF-α)、尿酸(UA)、白细胞介素-6(IL-6)、不良反应总发生率。结果:观察组临床总有效率96.83%高于对照组77.78%(P<0.05)。治疗后观察组膝关节屈曲活动度高于对照组(P<0.05)。治疗后观察组VAS评分、Leqensne评分、滑膜厚度均低于对照组(P<0.05)。治疗后观察组血清TNF-α、IL-6、ESR、UA水平均低于对照组(P<0.05)。观察组不良反应总发生率3.17%与对照组6.35%比较,差异无统计学意义(P>0.05)。结论:祛湿排浊汤可有效缓解持续性痛风性膝关节炎患者关节疼痛、肿胀等症状,改善膝关节活动度,减轻炎症反应,降低血沉、血尿酸含量,且未引发严重不良反应。

Objective:To analyze effect of Qushi Paizhuo decoction on improving knee motion and pain degree in the treatment of persistent gouty knee arthritis.Methods:126 patients with persistent gouout knee arthritis were selected for retrospective study.According to different treatment methods,the patients were divided into two groups with 63 cases in each group.The control group were given febusterat and diclofenac sodium.The observation group were treated with Qushi Paizhuo decoction on the basis of the control group.Clinical efficacy,knee flexion range of motion,VAS score,Leqensne score,synovial thickness,ESR,TNF-α,IL-6,UA,and total incidence of adverse reactions were compared between the two groups.Results:The total clinical effective rate of observation group 96.83%was higher than that in the control group 77.78%(P<0.05).The knee flexion motion of the observation group was higher than that in the control group after treatment(P<0.05).After treatment,VAS score,Leqensne score and synovial thickness of the observation group were lower than those in the control group(P<0.05).After treatment,the levels of TNF-α,IL-6,ESR and UA in the observation group were lower than those in the control group(P<0.05).The total incidence of adverse reactions in observation group 3.17%compared with that in control group 6.35%difference was not statistically significant(P>0.05).Conclusion:Qushi Paizhuo decoction can effectively relieve joint pain,swelling and other symptoms in patients with persistent gouty knee arthritis,improve knee motion,reduce inflammatory response,reduce erythrocyte sink and blood uric acid content,and not cause serious adverse reactions.
