支气管肺泡灌洗液中HSP90α、TSGF对肺部良、恶性疾病的诊断价值

Diagnostic Value of HSP90αand TSGF in Bronchoalveolar Lavage Fluid for Benign and Malignant Lung Diseases

目的 探讨支气管肺泡灌洗液(BALF)中热休克蛋白90α(HSP90α)、肿瘤特异性生长因子(TSGF)对肺部良、恶性疾病的诊断价值。方法 选取2020年1月至2022年4月商丘市中医院收治的经病理检查确诊的90例肺癌患者作为肺癌组,90例肺部良性疾病患者作为良性组。病理检查前两组均接受支气管肺泡灌洗治疗,采用酶联免疫吸附法(ELISA)检测两组BALF中HSP90α、TSGF水平;采用受试者工作特征(ROC)曲线分析其对肺部良、恶性疾病的诊断价值。结果 肺癌组BALF中HSP90α、TSGF水平均高于良性组(P<0.05);不同TNM分期、是否发生淋巴结转移的肺癌患者HSP90α、TSGF水平比较差异有统计学意义(P<0.05);不同年龄、性别、不同病灶直径大小、不同病理学特征的肺癌患者HSP90α、TSGF水平比较差异无统计学意义(P>0.05);ROC曲线分析显示,HSP90α鉴别诊断肺部良、恶性疾病的曲线下面积(AUC)为0.829,灵敏度为85.17%,特异度为77.03%;TSGF鉴别诊断肺部良、恶性疾病的AUC为0.779,灵敏度为73.28%,特异度为81.00%;二者联合鉴别诊断肺部良、恶性疾病的AUC为0.928,灵敏度为94.41%,特异度为89.75%。结论 BALF中HSP90α、TSGF水平对鉴别诊断肺部疾病性质及判断肺癌进展有一定的参考价值。

Objective To investigate the diagnostic value of heat shock protein 90α(HSP90α)and tumor specific growth factor(TSGF)in bronchoalveolar lavage fluid(BALF)for benign and malignant lung diseases.Methods A total of 90 lung cancer patients diagnosed through pathological examination admitted to Shangqiu Traditional Chinese Medicine Hospital from January 2020 to April 2022 were selected as the lung cancer group,and 90 patients with benign lung diseases were selected as the benign group.Both groups received bronchoalveolar lavage treatment before pathological examination,and enzyme-linked immunosorbent assay(ELISA)was used to detect the levels of HSP90αand TSGF in BALF of the two groups,and the diagnostic value of them identifying benign and malignant lung diseases was analyzed by receiver operating characteristic(ROC)curve.Results The levels of HSP90αand TSGF in BALF of lung cancer patients were higher than those in benign group(P<0.05).The levels of HSP90αand TSGF in BALF of lung cancer patients with different TNM stages and whether lymph node metastasis occurred were statistically different(P<0.05).There was no statistical difference in HSP90αand TSGF levels in BALF of lung cancer patients with different ages,genders,different lesion diameters and different pathological characteristics(P>0.05).ROC curve analysis showed the area under the curve(AUC)for HSP90αdifferential diagnosis of benign and malignant lung diseases was 0.829,the sensitivity was 85.17%,and the specificity was 77.03%.The AUC for TSGF differential diagnosis of benign and malignant lung diseases was 0.779,the sensitivity was 73.28%,and the specificity was 81.00%.The AUC for the combined differential diagnosis of benign and malignant lung diseases was 0.928,the sensitivity was 94.41%,and the specificity was 89.75%.Conclusion The levels of HSP90αand TSGF in BALF have certain reference value for differentiating and diagnosing the nature of lung diseases and judging the progression of lung cancer.
