基于扎根理论的临床护理路径对糖尿病视网膜病变患者的影响

Effects of Grounded Nursing Pathway in Patients with Diabetic Retinopathy

目的 探讨基于扎根理论的临床护理路径(CNP)对糖尿病视网膜病变(DR)患者的干预效果。方法 选取2020年1月至2022年12月郑州人民医院170例DR患者作为研究对象,按入院建档顺序分组,奇数为对照组,偶数为观察组,各85例。对照组接受常规护理,观察组接受基于扎根理论的CNP。比较两组血糖水平、治疗依从性、疾病认知度、预后效果、生活质量。结果 干预后观察组空腹血糖(FPG)、餐后2 h血糖(2 h PG)低于对照组,治疗依从性总分高于对照组,疾病认知度高于对照组,并发症发生率低于对照组(P<0.05);随访3个月后观察组视功能生存质量量表评分低于对照组(P<0.05)。结论 基于扎根理论的CNP能改善DR患者的血糖水平,提升其疾病认知度和治疗依从性,同时能减少患者并发症的发生,提高其生活质量。

Objective To investigate the effect of clinical nursing pathway(CNP)based on grounded theory in patients with diabetic retinopathy(DR).Methods A total of 170 patients with DR in Zhengzhou People’s Hospital from January 2020 to December 2022 were selected as research objects,and they were divided into groups according to the order of admission records,with odd numbers as the control group and even numbers as the observation group,with 85 patients in each group.The control group received routine nursing,and the observation group received CNP based on grounded theory.Blood glucose level,treatment compliance,disease awareness,prognostic effect and quality of life were compared between the two groups.Results After intervention,fasting blood glucose(FPG)and 2 hours postprandial blood glucose(2 h PG)in observation group were lower than those in control group,the total score of treatment compliance was higher than that in control group,the disease awareness was higher than that of control group and the complication rate of observation group was lower than that of control group(P<0.05).After 3 months of follow-up,the scores of observation group were lower than those of control group(P<0.05).Conclusion Based on grounded theory,CNP can improve the blood glucose level of patients with DR,improve their disease awareness and treatment compliance,reduce complications and improve their quality of life.
