结肠造口灌洗对永久性结肠造口术后患者生活质量的影响

The Effect of Colostomy Irrigation on the Quality of Life of Patients after Permanent Colostomy

目的:探讨结肠造口灌洗对永久性结肠造口术后患者生活质量的影响。方法:选取2019年5月至2022年5月于本院接受Miles术行永久性结肠造口的216例直肠癌患者,随机分为对照组和观察组,各108例。对照组术后接受自然排便传统造口袋护理,观察组接受规范化结肠造口灌洗护理,比较2组患者干预前、干预后3个月造口周围皮肤状况及患者生活质量。结果:干预3个月后,观察组患者造口变色、侵蚀、组织增生及DET总分均低于对照组,其生理、社会/家庭、情感、功能状况、附加关注及生活质量总分均比对照组高,P<0.05。结论:永久性结肠造口患者术后早期结肠造口灌洗有助于保护造口周围皮肤情况,提高患者生活质量。

Objective To explore the impact of colostomy irrigation on the quality of life of patients after permanent colostomy.Methods 216 rectal cancer patients who underwent Miles surgery for permanent colostomy in our hospital from May 2019 to May 2022 were randomly divided into control group and observation group,with 1o8 cases in each group.The control group received traditional pouch care for natural defecation after surgery,while the observation group received standardized colostomy irrigation care.The skin condition around the colostomy and the quality of life of the two groups of patients were compared before intervention and 3 months after intervention.ResultsAfter 3 months of intervention,the observation group patients had lower stoma discoloration,erosion,tissue hyperplasia,and total DET scores than the control group.Their physiological,social/family,emotional,functional status,additional attention,and total quality of life scores were all higher than the control group,P<o.05.Conclusion Early postoperative colostomy irrigation in patients with permanent colostomy can help protect the skin condition around the colostomy and improve the patient's quality of life.
