摘要
目的评估磁共振快速T2校正的多回波(Histo)序列测量肩关节冈上肌脂肪含量的应用价值。方法前瞻性分析2020年6月至2021年2月间在南京医科大学附属苏州医院治疗的81例肩关节疼痛患者临床及影像学资料。所有患者均在西门子Skyra 3.0T磁共振上接受如下MR序列检查:斜矢状位的快速自旋回波(fast-spin-echo FSE)T1加权序列成像的标准扫描;Histo序列和DIXON序列成像。由2名10年和12年肌骨磁共振诊断经验影像医师对患者在斜矢状位的T1加权影像进行Goutallier分级,分别评估Goutallier分级与Histo序列和DIXON序列定量结果间的相关性,并对Goutallier分级结果中不同脂肪浸润等级受检者冈上肌的Histo序列和DIXON序列定量测量结果间进行比较,并分析不同撕裂程度患者Histo序列脂肪定量结果的差异性。结果冈上肌脂肪化的Goutallier分级与Histo序列和DIXON序列定量结果具有相关性(ρ=0.799、0.765、0.599,P均<0.001)。Goutallier不同分级对应的Histo序列和DIXON序列定量结果差异均有统计学意义(P均<0.05)。不同冈上肌撕裂程度比较中,仅在冈上肌全层撕裂与冈上肌撕裂伴回缩两组间差异无统计学意义(Z=-1.557,P=0.120)。结论通过定量Histo序列测量冈上肌脂肪化的MR成像具有可行性,可更加精准地测量冈上肌脂肪化程度且与DIXON序列有较好的一致性。此外Histo序列定量结果对冈上肌不同撕裂程度有一定的提示作用。
Objective To evaluate the application value of magnetic resonance rapid T2-corrected multiple echo(Histo)sequence in measuring the fat content of shoulder joint supraspinatus muscle.Methods It was prospectively analyzed the clinical and imaging data of 81 patients with shoulder joint pain treated in Suzhou Hospital,Nanjing Medical University from June 2020 to February 2021.All patients underwent the following MR Sequences on Siemens Skyra 3.0T magnetic resonance:Standard T1-weighted sequence imaging with fast spin echo(FSE)in oblique sagittal position;Histo sequence and DIXON sequence imaging.Two radiologists with 10 and 12 years of experience in musculoskeletal magnetic resonance diagnosis performed Goutallier grading on T1-weighted images of patients in oblique sagittal position.It was evaluated the correlation between Goutallier grading and the quantitative results of Histo sequence and DIXON sequence,compared the quantitative results of Histo sequence and DIXON sequence of supraspinatus muscle of subjects with different levels of fat infiltration in Goutallier grading,and analyzed the difference of quantitative results of Histo sequence fat in patients with different tearing degrees.Results Goutallier grade of supratinatus muscle adipositosis was correlated with the quantitative results of Histo sequence and DIXON sequence(ρ=0.799,0.765,0.599,P<0.001).The quantitative results of Goutallier's Histo sequence and DIXON sequence corresponding to different grades were statistically significant(P<0.05).In the comparison of different degrees of supraspinatus muscle tear,there was no significant difference between the two groups only for total supraspinatus muscle tear and supraspinatus muscle tear with contraction(Z=-1.557,P=0.120).Conclusion It is feasible to measure the degree of supraspinatus muscle fat by quantitative Histo sequence,which can measure the degree of supraspinatus muscle fat more accurately and has better consistency with DIXON sequence.In addition,the quantitative results of Histo sequence have a certain suggestive effect on the different degrees of supraspinatus muscle tear.
