托法替布致不良反应文献分析

Literature Analysis of Adverse Drug Reactions Caused by Tofacitinib

目的通过对托法替布不良反应(ADR)的个案报道进行整理和分析,探讨其发生特点和处置方案,为临床安全合理用药提供参考。方法检索各数据库自建库以来至2022年9月收载的托法替布相关ADR的个案报道,并进行统计分析。结果检索到18篇共19例关于托法替布ADR的个案报道,以40~59岁患者的比例最高(9例,47.37%)。原患疾病以各种类型的关节炎报道最多(12例,占比63.16%)。用药时间在1~4个月内出现ADR的占比最高(10例,52.63%)。主要累及的器官/系统为免疫系统、皮肤和消化系统。结论临床应用托法替布的前4个月应密切防范ADR的发生,尤其是中老年患者以及儿童,并且对免疫系统的ADR提高警惕,确保用药安全。

OBJECTIVE The characteristics and treatment plan of adverse drug reactions(ADR)caused by tofacitinib were discussed,so as to provide reference for clinical safe and rational drug use.METHODS The case reports of tofacitinib-related ADR were collected in each database from creating database to September 2022 and analyzed statistically.RESULTS A total of 19 ADR cases were reported in 18 articles,with the highest proportion of patients aged 40-59 years(9 cases,47.37%).The primary diseases were mostly arthritis(12 cases,63.16%).The highest proportion of ADR occurred within 1-4 months(10 cases,52.63%).The main organs/systems involved are immune system,skin and digestive system.CONCLUSION In the first 4 months of clinical application of tofacitinib,ADR should be closely prevented,especially in middle-aged and elderly patients and children,and ADR of immune system should be vigilant to ensure the safety of drug use in patients.
