基于“肠道菌群-线粒体自噬”探讨“从脾论治”痛风性肾病

摘要 中医学“脾”与线粒体关系密切,脾虚常导致线粒体形态和功能异常。由于线粒体自噬是一种选择性的清除形式,而在脾虚诱导下产生的大量受损线粒体,不足以自我清除时,便极有可能导致线粒体自噬机制的紊乱。肠道菌群紊乱也是中医学“脾虚”的重要生物学基础。肠道菌群失调,线粒体自噬异常等生物稳态的失衡均会影响痛风性肾病的发展。脾虚运化失常,致湿、痰、瘀等“浊”邪堆积于肾,导致痛风性肾病。中药具有多靶点和全身治疗的优势,能够调节肠道菌群和包括线粒体自噬在内的多种线粒体生理过程。脾虚与肠道菌群和线粒体自噬之间的相互作用,建立了一个影响痛风性肾病发展的免疫代谢网络,这有可能是中药防治痛风性肾病的潜在靶点。 "Spleen"in traditional Chinese medicine is closely related to mitochondria,and Spleen deficiency often leads to abnormal morphology and function of mitochondria.Mitochondrial autophagy is a selective form of clearance,and when a large number of damaged mitochondria induced by Spleen deficiency are insufficient for self-clearance,it is very likely to lead to the disorder of mitochondrial autophagy mechanism.Intestinal flora disorder is also an important biological basis of Spleen deficiency in traditional Chinese medicine.The imbalance of intestinal flora and mitochondrial autophagy would affect the development of gouty nephropathy.Spleen deficiency is abnormal,which causes Dampness,Phlegm and blood stasis to accumulate in the Kidney,leading to gouty nephropathy.Traditional Chinese medicine has the advantages of multi-target and systemic treatment,and can regulate intestinal flora and various mitochondrial physiological processes including mitochondrial autophagy.The interaction between Spleen deficiency and intestinal flora and mitochondrial autophagy has established an immune metabolic network that affects the development of gouty nephropathy,which may be a potential target for traditional Chinese medicine to prevent and treat gouty nephropathy.
