Wang Yahong′s Experience in Treating Bicardiac Diseases

冠状动脉粥样硬化性心脏病(简称“冠心病”)患者常因情志致病,或由情志诱发、加重,或病后焦虑、胆怯。少阳主胆,少阳不舒,胆虚则容易胆怯、焦虑、数谋略而不决;胆郁则影响肝气疏泄导致急躁易怒,患者常伴有精神神经症状,多数双心疾病以少阳不舒、瘀阻心脉为主要病机。柴胡加龙骨牡蛎汤主治少阳不舒、郁热扰心之证,血府逐瘀汤为治疗气滞血瘀于胸中之证,二者合用治疗双心疾病,不仅增疏肝解郁、行气活血之力,且加强健脾利水,安神定志之功,对治疗冠心病伴焦虑、烦躁、抑郁、胆怯等神经精神疾病、失眠等疗效显著。临证不仅应着眼于冠心病还要注重改善患者的情绪、睡眠,使患者得到全方位的治疗,体现中医以人为本的基本观点。

Patients with coronary atherosclerotic heart disease(referred to as"coronary heart disease")often suffer from emotional illness,or are induced or aggravated by emotions,or are anxious and timid after illness.Shaoyang dominates the Gallbladder,and when Shaoyang Qi is stagnated,Gallbladder deficiency leads to timidness,anxiousness and indecisiveness;Depression of Gallbladder affects the release of Liver Qi and leads to impatience and irritability,and patients are often suffered mental and neurological symptoms.The main pathogenesis of most bicardiac diseases is Shaoyang Qi stagnation in Heart meridian.Chaihu plus Longgu Muli Decoction is mainly used to treat the syndrome of Shaoyang Qi stagnation and stagnation of Heat disturbing Heart,and Xuefu Zhuyu Decoction is used to treat the syndrome of Qi stagnation and blood stasis in the chest.The combination of the two can not only increase the power of soothing the Liver Qi,relieving depression,promoting Qi and blood circulation,but also strengthen the functions of invigorating the Spleen,inducing diuresis,calming the nerves as well as easing the mind,and has obvious curative effects on neuropsychiatric diseases such as coronary heart disease with anxiety,irritability,depression,timidity and insomnia.Clinical practice should not only focus on coronary heart disease but also pay attention to improving patients′mood and sleep condition,so that patients can get all-round treatment,which embodies the basic view of man-oriented in traditional Chinese medicine.
