仝小林院士态靶结合理论临证思考

Clinical Thinking and Practice Based on Academician Tong Xiaolin’s Status-target Combination

仝小林院士基于中医思维结合现代医学提出了“态靶因果”的临床辨治方略。“态”是疾病某阶段的整体概括,具有“状态、动态、态势”3层含义;靶分为“病靶”“态靶”及“标靶”。此文从阴阳五行学说的角度阐释“态”之内涵。其中五行强调了随空间变化的、相对稳定的势态,阴阳则描述了随时间变化的时态。同时,根据临床经验,结合仝院士四焦理论,笔者认为采用“四焦定位,态靶结合”的诊治模式,能够有效提高中医药的临床疗效。临证治疗时先根据四焦进行疾病定位,再识“态”、辨“态”,最后在调态方的基础上直接“打靶”,将定位、识态、打靶步步深入,在临床疗效上将获益良多。

Academician Tong Xiaolin proposed a clinical diagnosis and treatment strategy of“causal status and target”based on the thinking of traditional Chinese medicine combined with modern medicine.“Status”is the overall summary of a certain stage of the disease,with three meanings of“current state,dynamic state,and potential state”.Targets are divided into“disease target”“symptom target”and“manifestation target”.This paper explained the connotation of“status”from the perspective of the yin and yang five elements theory.The five elements emphasize the relatively stable potential that changes with space,while the yin and yang describe the tense that changes with time.Different diseases have different pathological characteristics and development trends,that is,the five elements of“status”.Different stages of the same disease are yin and yang tenses.The“status target”theory contains the connotation of the yin and yang five elements theory,and is the inheritance and innovation of the theory of Chinese medicine.At the same time,based on clinical experience combined with Academician Tong’s four-focal theory,we believes that the diagnosis and treatment mode of“four-focal positioning and state-target combination”can effectively improve the clinical effect of traditional Chinese medicine.In the case of clinical treatment,the disease is first located according to the four focal points,and then the“status”recognized,finally,the target is directly“targeted”on the basis of the state adjustment party.The positioning,state recognition,and targeting are deepened step by step,and good clinical efficacy has been achieved.
