清震汤联合甘露醇在高血压脑出血术后患者中的应用

Application of Qingzhen Decoction Combined with Mannitol in Patients with Hypertensive Cerebral Hemorrhage after Operation

目的:探讨清震汤联合甘露醇在高血压脑出血术后患者中的应用。方法:选取2019年1月—2022年10月大田县总医院收治的70例高血压脑出血术后患者作为研究对象。根据信封法将其随机分为对照组和联合组,各35例。对照组给予甘露醇治疗,联合组在对照组基础上给予清震汤。比较两组治疗前后中医症候积分、相关指标,不良反应及预后情况。结果:治疗后,两组嘴角歪斜、头晕头痛、半身不遂评分均明显降低,联合组嘴角歪斜、头晕头痛、半身不遂评分均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组肿瘤坏死因子-α(TNF-α)、同型半胱氨酸(Hcy)、神经元特异性烯醇化酶(NSE)水平均明显降低,神经生长因子(NGF)明显升高,联合组TNF-α、Hcy、NSE水平均低于对照组,NGF水平高于对照组,差异有统计学意义(P<0.05)。联合组优良率高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:清震汤联合甘露醇治疗高血压脑出血术后患者的临床效果更佳,有助于减轻患者临床症状,改善神经功能,提高预后效果。

Objective:To investigate the application of Qingzhen Decoction combined with Mannitol in patients with hypertensive cerebral hemorrhage after operation.Method:Seventy patients with hypertensive cerebral hemorrhage after operation treated in Datian General Hospital from January 2019 to October 2022 were selected as the study objects.According to envelope method,they were randomly divided into control group and combination group,35 cases in each group.The control group was given Mannitol treatment,and the combination group was given Qingzhen Decoction on the basis of control group.The scores of TCM symptoms,related indexes before and after treatment,adverse reactions and prognosis condition of the two groups were compared.Result:After treatment,the scores of oblique corners of the mouth,dizziness headache and hemiplegia were significantly reduced in both groups,and the scores of oblique corners of the mouth,dizziness headache and hemiplegia in the combination group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),homocysteine(Hcy)and neuron-specific enolase(NSE)were significantly decreased in both groups,while the levels of nerve growth factor(NGF)were significantly increased,the levels of TNF-α,Hcy and NSE in the combination group were lower than those in the control group,and the levels of NGF were higher than those in the control group,the differences were statistically significant(P<0.05).The excellent and good rate of the combination group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The clinical effect of Qingzhen Decoction combined with Mannitol in the treatment of patients with hypertensive cerebral hemorrhage after operation is better,which is helpful to reduce the clinical symptoms of patients,improve the neurological function and improve the prognosis effect.
