鼻内镜下鼻甲黏膜下部分切除术联合鼻中隔矫正术治疗变应性鼻炎伴鼻中隔偏曲的效果

Effect of Partial Submucosal Resection of Turbinate under Nasal Endoscopy Combined with Nasal Septum Construction in the Treatment of Allergic Rhinitis with Nasal Septum Deviation

目的:探讨鼻内镜下鼻甲黏膜下部分切除术联合鼻中隔矫正术治疗变应性鼻炎(AR)伴鼻中隔偏曲的临床效果。方法:选取2020年1月—2022年12月湖北科技学院附属浠水医院收治的98例AR伴鼻中隔偏曲患者作为研究对象,按随机数表法分为对照组和观察组,各49例。对照组予以鼻中隔矫正术治疗,观察组在对照组基础上加用鼻内镜下鼻甲黏膜下部分切除术治疗。比较两组术前、术后1个月鼻部症状评分、嗅觉功能、鼻通气功能及并发症发生情况。结果:观察组术后1个月鼻塞、鼻痒、喷嚏、流涕评分、鼻气道阻力低于对照组,Sniffin sticks嗅觉测试法中气味识别能力、气味辨别能力、气味感觉阈值及总分、鼻腔最小横截面积(NMCA)、鼻腔容积(NCV)高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:鼻内镜下鼻甲黏膜下部分切除术联合鼻中隔矫正术可提高AR伴鼻中隔偏曲治疗效果,减轻鼻部症状,加快嗅觉功能恢复,改善鼻腔通气能力,安全可靠。

Objective:To explore the clinical effect of partial submucosal resection of turbinate under nasal endoscopy combined with nasal septum construction in the treatment of allergic rhinitis(AR)with nasal septum deviation.Method:A total of 98 patients with AR and nasal septum deviation who admitted to Hubei University of Science and Technology Affiliated Xishui Hospital from January 2020 to December 2022 were selected as the research objects,they were divided into control group and observation group according to the random number table method,with 49 cases in each group.The control group was treated with nasal septum construction,while the observation group was treated with partial submucosal resection of turbinate under nasal endoscopy on the basis of the control group.The nasal symptom score,olfactory function,nasal ventilation function before and 1 month after operation and complications were compared between two groups.Result:The scores of nasal obstruction,nasal itching,sneezing,runny nose and nasal airway resistance in the observation group at 1 month after operation were lower than those in the control group,the odor recognition ability,odor discrimination ability,odor perception threshold and total score in Sniffin sticks olfactory test,nasal minimum cross-sectional area(NMCA)and nasal cavity volume(NCV)were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between two groups(P>0.05).Conclusion:Partial submucosal resection of turbinate under nasal endoscopy combined with nasal septum construction can improve the treatment effect of AR with nasal septum deviation,alleviate nasal symptoms,accelerate the recovery of olfactory function,improve nasal ventilation capacity,and it is safe and reliable.
