摘要
目的:探讨创伤性肋骨骨折后早期肺部并发症的危险因素。方法:回顾性分析2022年1—12月泰州市第四人民医院胸外科收治的80例创伤性肋骨骨折患者的临床资料。统计所有患者早期肺部并发症发生情况,收集所有患者的一般资料、既往史、临床特征、创伤严重程度评分(injury severity score,ISS)。分析所有患者早期肺部并发症发生情况,对创伤性肋骨骨折后早期肺部并发症的影响因素进行单因素及多因素分析。结果:80例创伤性肋骨骨折患者中,发生早期肺部并发症34例,发生率为42.5%。发生早期肺部并发症患者吸烟史、糖尿病史、肋骨骨折数量>5根、骨折处理时间>24 h占比,ISS评分均高于未发生早期肺部并发症患者,差异有统计学意义(P<0.05)。有吸烟史、有糖尿病病史、肋骨骨折数量>5根、高ISS评分是创伤性肋骨骨折后早期出现肺部并发症的独立危险因素(P<0.05)。结论:有吸烟史、有糖尿病史、肋骨骨折数量>5根、高ISS评分是创伤性肋骨骨折后早期肺部并发症的危险因素,以上因素的存在可能增加肺部并发症的风险,应在治疗中加以重视并采取相应的预防措施。
Objective:To explore the risk factors of early pulmonary complications after traumatic rib fracture.Method:The clinical data of 80 patients with traumatic rib fracture admitted to the Department of Thoracic Surgery of Taizhou Fourth People's Hospital from January to December 2022 were retrospectively analyzed.The occurence condition of early pulmonary complications in all patients was counted,and the general data,past history,clinical features,and injury severity score(ISS)of all patients were collected.The occurence condition of early pulmonary complications in all patients was analyzed,and the influencing factors of early pulmonary complications after traumatic rib fracture were analyzed by univariate and multifactorial analysis.Result:Among 80 patients with traumatic rib fracture,34 patients had early pulmonary complications,with the incidence was 42.5%.The proportions of smoking history,diabetes history and number of rib fractures>5,fracture treatment time>24 h,ISS score in patients with early pulmonary complications were higher than those in patients without early pulmonary complications,the differences were statistically significant(P<0.05).History of smoking,history of diabetes,number of rib fractures>5 and high ISS score were independent risk factors for early pulmonary complications after traumatic rib fractures(P<0.05).Conclusion:Smoking history,history of diabetes,number of rib fractures>5,and high ISS score are risk factors for early pulmonary complications after traumatic rib fractures.The presence of the above factors may increase the risk of pulmonary complications and should be given attention and corresponding preventive measures in treatment.
