辨证论治辅助机器复位治疗耳石症临床观察

Treatment Based on Syndrome Differentiation in the Assisted Machine Reduction of Otolithiasis

目的探索辨证论治辅助机器复位治疗耳石症的效果。方法62例耳石症随机平均分为对照组和干预组。对照组予机器复位,干预组予机器复位加辨证治疗。2组分别于治疗前,及治疗后1 d、1周、2周评估躯体(P)、功能(F)、情绪(E)指数,记录眩晕残障量表(DHI)评分。随访6个月,比较2组耳石症复发、残余头晕患者出现率。结果治疗1周后与治疗前比较,2组DHI评分均有明显下降,且干预组下降更明显(P<0.05);2周后,2组DHI评分均显著下降(P<0.01)。干预组复发率在第2个月、第4个月、第6个月明显低于对照组(P<0.05);对照组残余头晕出现率在第2周和第1个月高于干预组(P<0.05)。结论辨证论治加机器复位治疗耳石症可提高效果,降低残余头晕病例,减少复发。

Objective To explore the curative effect of treatment based on syndrome differentiation in the assisted machine reduction of otolithiasis.Methods Sixty-two cases of otolithiasis were randomly divided into control group and intervention group.The control group was given the machine reduction,and the intervention group was given machine reduction and treatment based on syndrome differentiation.The physical(P),functional(F)and emotional(E)indexes of the two groups were evaluated before treatment and 1 day,1 week and 2 weeks after treatment,respectively,and the score of vertigo Disability Scale(DHI)was recorded.After 6 months of follow up,the incidence of recurrent otoliths and residual dizziness were compared between the two groups.Results After one week of treatment,DHI score in both groups decreased significantly compared with before treatment,and the decrease was more obvious in the intervention group(P<0.05).After two weeks,DHI scores in both groups decreased significantly(P<0.01).The recurrence rate in the intervention group was significantly lower than that in the control group at 2 months,4 months and 6 months(P<0.05).The incidence of residual dizziness in the control group was higher than that in the intervention group at the second week and the first month(P<0.05).Conclusion Treatment based on syndrome differentiation in the assisted machine reduction can improve the curative effect of otolithiasis,reduce the cases of residual dizziness and reduce the recurrence.
