腹腔镜结直肠癌根治术与开腹手术治疗结直肠癌的效果比较

Effect Comparison of Laparoscopic Radical Resection and Laparotomy in Treatment of Colorectal Cancer

目的探讨结直肠癌患者采用腹腔镜根治术与传统开腹手术治疗的临床疗效。方法选取2021年1月-2022年12月射阳县人民医院收治的120例结直肠癌患者为研究对象,以随机数字表法为主,共分为常规组(n=60)和研究组(n=60),常规组采用传统开腹手术,研究组采用腹腔镜结直肠癌根治术手术,分析对比两组患者治疗效果。结果研究组并发症发生率低于常规组,差异有统计学意义(P<0.05)。研究组手术时间、住院时间、术中出血量、术后下床时间、肠道排气时间及正常饮食时间均优于常规组,差异有统计学意义(P<0.05);术后,研究组VAS分数均低于常规组,差异有统计学意义(P<0.05);研究组生活质量各维度分数与常规组对比更高,差异有统计学意义(P<0.05);治疗后,研究组CD3^(+)、CD4^(+)与常规组对比更高,研究组CD8^(+)指标与常规组对比更低,差异有统计学意义(P<0.05)。结论结直肠癌患者采用腹腔镜根治术治疗后的效果与开腹手术对比更理想,有利于患者术后恢复和生活质量的改善,有效降低并发症,缓解患者的疼痛程度,值得推广。

Objective To explore clinical effect of laparoscopic radical surgery and traditional laparotomy in treatment of colorectal cancer.Methods The paper collected 120 patients with colorectal cancer in Sheyang County People's Hospital from January 2021 to December 2022,and divided them into routine group(60 cases)and study group(60 cases).the former was traditional laparotomy and the latter with laparoscopic radical colorectal cancer surgery.Treatment effect was analyzed.Results Incidence of complications in study group was(1.67%),lower than routine group(11.66%),P<0.05;Study group had better surgical time,length of stay,intraoperative bleeding volume,postoperative bedtime,intestinal exhaust time and normal eating time than routine group,P<0.05;After surgery,VAS scores in study group were lower than routine group,P<0.05.Study group had higher scores in all dimensions of quality of life than routine group,P<0.05.After treatment,CD3+and CD4+levels in study group were higher than routine group,while CD8+indicators in study group were lower thant routine group,P<0.05.Conclusion Laparoscopic radical surgery can achieve more ideal effect than laparotomy for colorectal cancer patients,which is beneficial for postoperative recovery and improvement of life quality,can reduce complications and alleviate pain of patient effectively,which is worthy of promotion.
