腹腔镜结肠癌切除联合完整结肠系膜切除在老年结肠癌手术中的应用价值分析

Application Value Analysis of Laparoscopic Colon Cancer Resection Combined with Complete Mesocolon Resection for Elderly Colon Cancer Surgery

目的探讨对老年结肠癌行手术治疗的患者实施腹腔镜结肠癌切除联合完整结肠系膜切除方案的临床效果。方法选取丹阳市中医院普外科在2018年1月-2023年1月接收的行手术治疗的58例老年结肠癌患者,分组模式以简单随机法进行,对照组29例以开腹完整结肠系膜切除治疗,观察组29例,以腹腔镜结肠癌切除联合完整结肠系膜切除术治疗,对两组治疗效果进行比较。结果两组在手术用时,术中出血量方面差异不明显(P>0.05);观察组淋巴结清扫数量多于对照组,术后排气时间,住院时间显著少于对照组(P<0.05);观察组并发症发生率明显少于对照组(P<0.05)。结论对于行手术治疗的老年结肠癌患者,以腹腔镜结肠癌切除联合完整结肠系膜切除术方案进行治疗,获得满意效果,术后恢复周期短,安全性高,值得临床推广。

Objective To investigate clinical effect of laparoscopic colon cancer resection combined with complete mesocolon resection for elderly patients with colon cancer.Methods The paper chose 58 elderly patients with colon cancer with surgical treatment in General Surgery Department of DanYANG City Hospital of Traditional Chinese Medicine from January 2018 to January 2023,and divided them into groups with simple and random method.Control group(29 cases)was treated with open complete mesocolectomy,and observation group(29 cases)with laparoscopic colon cancer resection combined with complete mesocolectomy.Curaitve effect between two groups was compared.Results There was no significant difference in intraoperative bleeding between two groups during surgery,P>0.05;number of lymph node dissection(18.46±2.42)in observation group was higher than control group,postoperative exhaust time(3.88±0.58)days and length of stay(7.56±0.88)days were significantly lower than control group,P<0.05.There were 1 case of lymphatic fistula and 1 case of incomplete Bowel obstruction in observation group,incidence rate of 6.90%was significantly lower than control group(P<0.05).Conclusion Laparoscopic colon cancer resection combined with complete mesocolon resection cana achieve ideal effect with short postoperative recovery period and high safety for elderly patients with colon cancer.
