摘要
目的探讨血清白蛋白、血红蛋白、胱抑素C与卒中后认知障碍(PSCI)的关系。方法回顾性分析2019年1月—2022年8月本院收治的135例缺血性脑卒中患者的临床资料,根据3个月后是否存在认知障碍分为PSCI组51例和非PSCI组84例,比较2组患者临床资料,采用logistic回归分析影响PSCI的因素,应用ROC曲线分析血清学指标预测PSCI的价值,应用Pearson相关性分析血清学指标与MoCA评分的相关性。结果PSCI组患者年龄、高血压患病率、糖尿病患病率、胱抑素C水平显著高于非PSCI组(P<0.05),受教育程度、血红蛋白、血尿酸、白蛋白水平显著低于非PSCI组(P<0.05);logistic回归分析显示,年龄、受教育程度、高血压、糖尿病、血红蛋白、白蛋白、胱抑素C是卒中后认知障碍的主要影响因素(P<0.05);ROC曲线分析结果显示,血红蛋白、白蛋白、胱抑素C预测卒中后认知障碍的AUC为0.668、0.756、0.773;Pearson相关性结果显示,PSCI患者血红蛋白、白蛋白与MoCA评分成正相关关系(r=0.575、0.532,P<0.05),胱抑素C与MoCA评分成负相关关系(r=-0.583,P<0.05)。结论白蛋白、血红蛋白、胱抑素C与卒中后认知障碍关系密切,可以用于早期评估PSCI风险。
Objective This paper aims to investigate the relationship between serum albumin,hemoglobin,cystatin C and post-stroke cognitive impairment(PSCI).Methods The clinical data of 135 patients with ischemic stroke admitted to our hospital from 2019 to 2022 were retrospectively analyzed,the patients were divided into PSCI group(51 cases)and non-PSCI group(84 cases)according to the existence of cognitive impairment 3 months later,and the clinical data of the two groups were compared.Logistic regression was used to analyze the factors affecting PSCI,ROC curve was used to analyze the value of sero⁃logical indicators in predicting PSCI,and Pearson correlation analysis was used to analyze the correlation between serological indicators and MoCA score.Results Age,prevalence of hypertension,prevalence of diabetes and cystatin C level in PSCI group were higher than those in non-PSCI group(P<0.05)and the levels of education,hemoglobin,blood uric acid and albumin in PSCI group were lower than those of non-PSCI group(P<0.05).Logistic regression analysis showed that age,education level,hypertension,diabetes,hemoglobin,albumin and cystatin C were the main influencing factors of post-stroke cognitive impairment(P<0.05).ROC curve analysis results showed that the AUCs of hemoglobin,albumin and cystatin C in predicting cognitive impairment after stroke were 0.668,0.756 and 0.773.Pearson correlation showed that hemoglobin and albumin in PSCI patients were positively correlated with MoCA score(r=0.575,r=0.532,P<0.05)and cystatin C was neg⁃atively correlated with MoCA score(r=-0.583,P<0.05).Conclusion Albumin,hemoglobin and cystatin C are closely related to post-stroke cognitive impairment and can be used for early assessment of PSCI risk.
