2种品牌粪便血红蛋白和转铁蛋白胶体金免疫层析试纸检测限和检出率比较

Limits and rates of detection for hemoglobin and transferrin in fecal samples:comparison of colloidal gold immunochromatographic test papers with two different brands

摘要 目的观察2种品牌胶体金免疫层析试纸检测粪便血红蛋白和转铁蛋白的检测限及其对临床粪便样本检测的阳性率。方法采用2种胶体金试纸同时检测倍比稀释的血红蛋白和转铁蛋白标准液,同一浓度平行检测10次,检测结果均为阳性的最低稀释浓度即为该试纸的检测下限。采用2种试纸同时检测连续送检的268例粪便样本,比较粪便血红蛋白单检及其联合转铁蛋白检测的阳性率。结果A、B2种品牌的血红蛋白和转铁蛋白检测下限与说明书标示值一致,分别为150 ng/mL和40ng/mL。2种品牌血红蛋白试纸检测粪便样本阳性率分别为42.5%、28.4%,联合转铁蛋白检测的阳性率分别为60.1%、55.2%。结论即使不同品牌胶体金试纸血红蛋白和转铁蛋白的检测下限相同,但单检或联检的阳性率有显著差异,联检的阳性率高于单检。 ObjectiveTo investgate the sensitivity and positive rate of two brands of test paper reagents with colloidal gold immunochromatography for the detection of hemoglobin and transferrin in feces samples from cancer patients,Methods The double-diluted standards of hemoglobin and transferrin solution with different concentrations were detected simultaneously by using the test paper reagents with colloidal gold.The parallel detections for same concentration were performed for ten times concurrently.The lowest dilution concentration with positive results for all the samples was considered as the lower limit of detection of the test paper.Two kinds of test strips were used to detect 268 feces samples from cancer patients and the positive rates of single test for hemoglobin and the tests combined with transferring were combined.Results The limits of hemoglobin and transferrin detected by brands A and B were 150 ng/mL and 40 ng/mL respectively,which were consistent with those stated in the instructions from regent kits.The positive rates of the two brands of hemoglobin test paper were 42.5%and 28.4%,respectively,and the positive rates of the combined transferrin test were 60.1%and 55.2%,respectively.The positive rate of combined detection for hemoglobin and transferrin increased.Conclusion Although the lower limits of detection for hemoglobin and transferrin by different brands of colloidal gold test paper was the same,the positive rates of single test or combined tests were significantly different,and the positive rates of combined tests should be higher than those of single test.The simultaneous detection for hemoglobin and transferrin in the feces appeared to improve the diagnostic accuracy for gastrointestinal bleeding,presumably without raising"false-positive"reactions.
