鼓室灌注联合静脉注射地塞米松治疗突发性耳聋的效果及其对听力状况的影响

Therapeutic effect of tympanic perfusion combined with intravenous injection of dexamethasone on sudden deafness and its impact on hearing status

目的:分析鼓室灌注联合静脉注射地塞米松治疗突发性耳聋的效果及其对听力状况的影响。方法:抽取2017年1月至2020年12月联勤保障部队第九九〇医院收治的突发性耳聋患者60例,按随机数字表法分为研究组与对照组,每组30例。对照组采用静脉注射地塞米松治疗,研究组在对照组基础上采用鼓室灌注地塞米松治疗。比较两组临床疗效、症状改善情况、纯音听阈值及不良反应发生率。结果:研究组总有效率(63.33%,19/30)与对照组(53.33%,16/30)比较差异未见统计学意义(P>0.05)。研究组耳鸣消失时间、眩晕消失时间、听力恢复时间短于对照组(P<0.05)。治疗后,研究组500、1000、2000、4000 Hz纯音听阈值均低于对照组(P均<0.05)。研究组不良反应发生率(3.33%,1/30)与对照组(13.33%,4/30)比较差异未见统计学意义(P>0.05)。结论:鼓室灌注联合静脉注射地塞米松治疗突发性耳聋效果显著,可减轻临床症状,改善听力状况,且安全性良好。

Objective To analyze the efficacy of tympanic perfusion combined with intravenous injection of dexamethasone in the treatment of sudden deafness and its impact on hearing status.Methods Sixty patients with sudden deafness treated in No.990 Hospital of the Joint Logistic Support Force from January 2017 to December 2020 were selected,and they were divided into study group and control group according to random number table method,with 30 cases in each group.The control group was given intravenous dexamethasone injection,and the study group was given tympanic perfusion with dexamethasone based on the treatment of the control group.The curative effect,symptom improvement,pure tone hearing threshold and incidence of adverse reactions were compared between the two groups.Results There was no statistically significant difference in the total effective rate between the study group(63.33%,19/30)and the control group(53.33%,16/30),P>0.05.The tinnitus disappearance time,vertigo disappearance time and hearing recovery time in the study group were shorter than those in the control group(P<0.05).After treatment,the threshold values of pure tone of 500 Hz,1000 Hz,2000 Hz and 4000 Hz in the study group were lower than those in the control group(all P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the study group(3.33%,1/30)and the control group(13.33%,4/30),P>0.05.Conclusions Tympanic perfusion combined with intravenous injection of dexamethasone has a significant effect in the treatment of sudden deafness,which can alleviate clinical symptoms and improve hearing status,with good safety.
