脓毒症相关性脑病脑功能监测的研究进展

摘要 脓毒症相关性脑病(SAE)是脓毒症在中枢神经系统的并发症,其发病率及病死率较高,由于其发病机制及病理生理过程不清楚,目前暂无统一诊断标准。脑功能监测作为辅助诊断方法逐渐在神经重症领域开展,临床多采用两种及以上监测方法对患者病情进行评估,对指导临床干预、评估患者预后有重要意义。目前临床多通过患者的症状、体征并结合脑功能监测对SAE患者进行诊断。SAE所致脑缺血缺氧的发生和脑保护一直是研究和探讨的热点,脑功能监测在重症医学领域对指导个体化选择镇静药物、脑保护药物及病情评估起指导作用,合理选择监测方法对SAE的诊治起到降低医疗费用、提高医疗服务的作用,更对SAE在分子生物学领域的研究起到铺垫作用。 Sepsis-associated encephalopathy(SAE)is a complication of sepsis in central nervous system,the incidence and mortality of which are high.Because its pathogenesis and pathophysiology are still unclear,there is no unified diagnostic standard at present.As an auxiliary diagnostic method,brain function monitoring has been gradually carried out in the field of neurocritical diseases.Two or more monitoring methods are usually used to evaluate the severity of the patients′condition,which is of great significance for guiding the clinical intervention and evaluating the prognosis of the patients.At the moment,the clinical diagnosis of SAE is mainly based on the symptoms and signs of the patients combined with brain function monitoring.The occurrence of cerebral ischemia and hypoxia and brain protection caused by SAE has always been a hot point of the research and discussion.Brain function monitoring plays a directing role in guiding the individual selection of sedative drugs,brain protective drugs and disease assessment in the field of critical care medicine.Reasonable choice of monitoring methods can reduce medical costs,improve medical services for the diagnosis and treatment of SAE,and will pave the way for the research of SAE in the field of molecular biology.
